| Objective To compare the characteristics of Upper urinary calculi with type 2 diabetes mellitus and Upper urinary calculi without diabetes,to understand the characteristics of Upper urinary calculi with type 2 diabetes mellitus and treatment considerations.It provides the basis choice of treatment for the patients with upper urinary tract calculi with type 2 diabetes.Methods We used retrospective case-control study.Patients with upper urinary tract stones in Chenzhou First People’s Hospital were investigated from January 2014 to June 2015.The data of patients were recorded and analyzed.660 patients were classfied to Upper urinary calculi with type 2 diabetes mellitus(Case group 172 cases)and Upper urinary calculi without diabetes(Control group of 488 cases).We grouped them according to need.All data were statistically analyzed by Sppss19.0 statistical software and got the results.Results(1)There was no significant difference in the number of hospital days、number of days of anti-infection、PT and INR、stone maximum diameter、hydronephrosis、urine culture、preoperative plateletcount、gender and age between case group and control group(p>0.05).(2)Case group and control group pain,kidney area percussion pain,ureteral tenderness,gross hematuria difference was statistically significant(P<0.05).(3)There were no significant differences in urinary tract irritation,enhancement of antimicrobial agents,ICU,renal insufficiency and sepsis in case group and control group(p>0.05).(4)There was no significant difference in serum creatinine between the case group and the control group before and after operation(p>0.05);The difference of serum creatinine between preoperative and postoperative patients was statistically significant(P<0.05);There was no significant difference in serum creatinine between preoperative and postoperative groups(p>0.05).(5)There were statistically significant differences in the multiple surgery between the case group and the control group(P<0.05);There were no statistically significant differences in the causes of multiple operations between the case group and the control group(p>0.05).(6)There was significant difference in preoperative anemia between case group and control group(P<0.05);There was no significant difference in the degree of anemia between the two groups(P>0.05).(7)There was no significant difference in blood routine white blood cell count between case group and control group(P>0.05);There was no significant difference in blood routine white blood cell count between preoperative and postoperative patients(P>0.05);There was no significant difference in blood routine white blood cell count between preoperative and postoperative Control group(P>0.05).Conclusions(1)Upper urinary tract stones combined with type 2 diabetes clinical manifestations may not be typical,and not be consistent with the severity of the disease.(2)Urinary stones on the merger in patients with type 2 diabetes in urinary tract infections,kidney damage,anemia,blood coagulation function change more likely.Surgery tolerance may fall and may require longer time in hospital.(3)Upper urinary tract stones with type 2 diabetes patients may be more complex,more prone to surgical complications,the possibility of secondary surgery increased.(4)Urinary stones combined with type 2 diabetes on stone volume is relatively small,hydronephrosis is relatively light,the incidence of sepsis may not rise significantly,need further research,these need to be further confirmed... |
