| Objective:The purpose of this study was to compare the effects of repetitive transcranial magnetic stimulation(TMS)on the cognitive function of patients with cerebral infarction at high frequency.Methods:(1)We collected from early January 2016 to the end of May 2017 in our hospital rehabilitation department inpatient rehabilitation therapy for 89 cases diagnosed with cerebral infarction and functional activity of unilateral limb dysfunction,cognitive impairment patients as the research object.According to the number of all the above randomized patients were randomly divided into control group and treatment group,the control group was given cognitive rehabilitation training,treatment group on the basis of the control group,and then subdivided into 5HZ,10 HZ,20HZ corresponding to three different frequency stimulation for the treatment of patients with high frequency repetitive transcranial magnetic stimulation.(2)Before the treatment and the end of the rehabilitation treatment,the cognitive function of patients with mini mental state examination summary assessment of cognitive function,Fugl-Meyer evaluation of hemiplegic limbs,the modified Barthel index assessment of the ability of daily life to all patients with cerebral infarction.(3)At the beginning of the treatment and at the end of the course of treatment,the event related potential P300 of the patients was examined by using the brain evoked potential instrument.(4)Comparison of counting data use chi-square test,the measurement data is expressed as mean ± standard deviation the comparison between groups use independent samples t test;paired sample t test was used in group comparison.P<0.05 was considered to be statistically significant,P < 0.01 showed significantdifference.Results:(1)The control group and the treatment group after treatment MMSE score were higher than before treatment,the difference was statistically significant(P <0.01),and the frequency in the treatment group were 10 HZ and 20 HZ in patients with MMSE scores than the control group(P < 0.05),while the frequency of 5HZ MMSE compared with the control group difference no statistical significance(P > 0.05).(2)After treatment,the amplitude of P300 in the control group and the two groups of the treatment group was increased,and the latent period was shortened.The P300 results of 10 HZ and 20 HZ were all better than those of the control group(P <0.01 or P < 0.05),but the improvement effect of 10 HZ treatment group was more obvious than that of 20 HZ,but it could not be proved that the effect of 5HZ treatment group was different from that of control group.(3)According to the Hamilton anxiety scale,the results showed that the high frequency stimulation of 10 HZ and 20 HZ could significantly improve the emotional state of the patients.(4)Barthel index in each group was higher than that before the treatment,and the treatment group was better than the control group,and it had strength dependence in a certain frequency range.Conclusion:(1)High frequency rTMS combined with cognitive rehabilitation training can further improve cognitive function in patients with cerebral infarction,and help to restore the function of the hemiplegia side limbs and improve the daily living ability.(2)Under the same condition,the effect of high frequency stimulation of 10 HZ on cognitive function after cerebral infarction is better than that of 5HZ and 20 HZ.(3)High frequency rTMS in a certain range is safe and effective for the rehabilitation of patients with cerebral infarction,and it is worth promoting in clinical practice. |
