A Study On The Vertical Growth Changes In Patients With Mean Facial Angle With Skeletal Class Ⅰ And Ⅱ Malocclusion Mandibular Retrution At Different Developmental Stages

Purpose:Using the cervical skeletal staging method to cross-sectionally study the growth of facial height of patients with skeletal Class I and II malocclusion,mean facial angle and mandibular retrusion,and compare the difference of growth,so as to provide a reference for the clinical orthodontic treatment of vertical control of the patients with skeletal Class II malocclusion,mean facial angle and mandibular retraction.Method:Cephalometric radiographs of initial-visit patients 240 cases aged 8 to 18 were collected from January 2015 to December 2016 at the orthodontic department of Nanchang University Affiliated Stomatological Hospital.According to the ANB angle,GOGN-SN angle and Cervical skeletal age were divided into two groups: skeletal Class I malocclusion group(120 cases in total,30 cases in phase QCVM1 to QCVM4),and skeletal Class II malocclusion group(120 cases in total,30 cases in phase QCVM1 to QCVM4),the vertical indicators of the cephalometric radiographstabs selected were measured by Winceph 8.0 software(N-Me,N-ANS,ANS-Me,S-Go,Ar-Go,N-ANS/N-Me,ANS-Me/N-Me,S-Go /N-Me).Each measurement item was measured 3 times and averaged.All measurement data was recorded in SPSS 17.0software,the variance analysis was carried out on the measurement items of the two groups respectively.The data statistically different was compared with the LSD-t test to compare the growth trend of facial height in different cervical skeletal age.Two independent sample t-tests were performed on the measurement items of different cervical skeletal ages in the two groups,and the differences in growth and development under the different bone factors in the same cervical skeletal age were compared.ANOVA was used to analyze the D-value of the facial height of different stages of cervical skeletal age and the difference of growth volume under different bone factors.Results:1.The increase of N-ANS/N-Me,ANS-Me/N-Me and S-Go /N-Me was not statistically significant(P>0.05)from QCVM1 to QCVM4.The increase of N-Me,S-Go,Ar-Go ware statistically significant(P<0.05),and the difference between N-Me in QCVM1 and QCVM3 was statistically significant(P<0.01);the difference between S-Go in QCVM1 and QCVM4 was statistically significant(P<0.01);Ar-Go was in QCVM1 and QCVM3(P<0.05),QCVM1 phase and QCVM4 phase(P<0.01),QCVM2 and QCVM4(P<0.01)were statistically significant.2.There was a significant difference in N-Me and 、ANS-Me between QCVM1 phase and QCVM3 stage QCVM1 stage and QCVM4 stage in the mandibular retraction group of Class Ⅱ(P<0.05).There were significant differences in S-Go between QCVM1 and QCVM3 、QCVM1 and QCVM4 、QCVM2 and QCVM3 、QCVM2 and QCVM4(P<0.05).The difference of Ar-Go in QCVM1 and QCVM3 、QCVM1 and QCVM4 、 QCVM2 and QCVM4 was statistically significant(P <0.05).The difference of S-Go / N-me in QCVM2 and QCVM4 was statistically significant(P<0.05).3.There was no significant difference in all items measured between QCVM stage 1,QCVM stage 2,QCVM stage 3,QCVM stage 4 skeletal I mean angle group and skeletal II mean angle mandibular retrusion group(P>0.05).4.The difference between QCVM2 and QCVM1 in the homogenized mandibular retraction group of class Ⅱ showed an increasing trend compared with that in the group of bone type I mean angle.There was a decreasing trend in the ratio of N-ANSI N-MeN-ANS / N-Me to the ratio of N-MeS-Me / N-Me-Go / N-Me,and only the difference of N-ANS / N-Me was significant(P < 0.05).5.There was no significant difference in the difference between QCVM3 stage and QCVM2 stage and in the difference between QCVM4 stage and QCVM3 stage in skeletal class I mean angle group and skeletal class II mean angle mandibular retrusion group(P>0.05).Conclusions:The growth of N-ANS from QCVM1 to QCVM3 in the mandibular retraction group of type ⅱ of bone is relatively rapid,and the growth is larger than that in the group of homogenous mandibular retraction.ANS-Me grew faster in QCVM2 to QCVM3 than in group QCVM3.N-Me grows uniformly in QCVM1 to QCVM3 and slows down in QCVM4.S-Go、Ar-Go grows more slowly from QCVM1 to QCVM2 than from QCVM2 to QCVM3,but it is still smaller than the osteogenic class I homogeneous angle group.ANS-Me / N-Me is slightly larger than N-ANS / N-Me.
