| Background:China has entered an aging society,and the fall is one of the main threats to the elderly.The osteoporotic vertebral compression fracture of the elderly has become a common disease.Percutaneous kyphoplasty(PKP)is one of the main treating methods OVCF.Since the classical PKP through hibateral approach has the shortages of long operation time and excessive radiation exposure,unilateral approach find its increasingly wide application in clinic.But there are some safety concerns such sa breaking the medial wall of the pedicle of vertebral arch and injury of spinal cord or nerve root through unilateral approach,especially in thoracolumbar segment where pedicle diameter is thinner and the angle between the long axis of pedicle and the sagittal plane of vertebral body is comparatively smaller.Therefore,avoiding pedicle medial wall and nerve injury by successful accessing to unilateral puncture path,reaching effective target and making sufficient dispersion of bone cement in vertebral body is the critical point to success of unilateral approach PKP.Objective:To assess the security and efficacy of PKP by unilateral extra-pedicle of vertebral arct approach in the treatment of senile thoracolumbar OVCF.Methods:The date of 40 elderly patients(48 vertebras)with OVCF undergone PKP by unilateral extra-pedicle of vertebral arct approach were collected for this retrospective study.peroperative and postoperative vertebral haight,Cobb angle and the diffusion and leakage of bone cement were recorded and analyed.Visual analogue scale(VAS)and Oswestry Dysfunction Index(ODI)were used to evaluate the curative effect.Results:40 patients were all successfully operated.The vertebral anterior height,Cobb angle,VAS and ODI scores were significantly improved(P<0.05).All bone cement in the vertebras exceeded the central line,and no spinal cement leakage were discovered.Conclusion:PKP by unilateral extra-pedicle of vertebral arct approach in treating thoracolumbar OVCF is proved to be safe,effective and easy to operate. |
