Quality Of Life Assessed In Patients With Nonvalvular Atrial Fibrillation And The Effect Of Left Atrial Appendage Occlusion

Objective:To investigate the quality of life(QOL)in patients with nonvalvular atrial fibrillation(NVAF)and the influence of left atrial appendage occlusion(LAAO).Materials and Methods:100 NVAF patients,including 11 cases of LAAO,were selected in Cardiology Department of Zhongda hospital from September 2014 to June 2016.Another 100 persons without AF in the same age and gender were matched as control group.The anxiety and depression were evaluated by the Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)respectively,and the QOL was evaluated by the 36-Short-Form health survey(SF-36).After three months,the LAAO patients were evaluated by the 36-Short-Form health survey again.All of the collected data were analyzed by SPSS 13.0.Results:(1)NVAF patients had a lower QOL scores than the control group in eight different dimensions(63.04±8.51vs.69.72±7.69,58.25±18.14vs.69.70±15.12,66.110±12.52vs.71.30±12.18,57.95±13.91vs.66.25±12.78,55.65±11.93vs.62.400±12.28,68.69±14.85vs.75.80±11.09,67.69±21.94vs.72.36±15.74,70.86±15.90vs.77.96±9.56).All showed significant difference except the seventh one(P<0.05).(2)There are 42%NVAF patients had emotional disorders,and 7 persons(7%)had emotional disorders in the control group.There has a significant difference between the two groups about the emotional disorder(P<0.05).(3)The patients with NVAF were divided into two groups(emotional disorders group and normal emotional group),there was significant difference between the two groups in the QOL scores(65.40±8.48vs.59.79 ±7.50,62.93±17.04vs.51.79±16.92,69.05±12.90vs.62.02±10.88,62.14±13.97vs.54.91±13.12,58.79±12.82vs.51.43±8.92,72.05±14.15vs.64.57±14.79,72.44±19.87vs.62.72±22.34,74.28±16.04vs.66.14±14.63)(P<0.05).(4)After LAAO,the QOL scores in NVAFpatients were improved(59.09±6.89vs.61.82±7.26,54.55±15.08vs.75.00±15.81,62.27±6.47vs.68.64±6.74,63.18±10.31vs.68.64±8.69,51.36±8.97vs.67.27±8.47,73.76±12.46vs.86.89±4.49,75.78±15.55vs.78.81±16.80,76.36±6.31vs.85.46±8.63).and five dimensions showed significant difference(P<0.05).Conclusions:NVAF patients showed higher incidence of anxiety or depression and low QOL than the persons without AF.Also,the anxiety or depression had negative influence on the QOL of NVAF patients.The LAAO can improve the QOL of NVAF patients.
