[Objectives] The blood volume of pregnant women increased by about 40%-50% compared with non-gestational age.The increase was mainly due to plasma volume and blood cells were relatively small.Therefore,the blood was in a certain dilution state,which could lead to iron deficiency anemia Deficiency Anemia,IDA).This study was to analyze the serum ferritin levels in the middle and late stages of pregnancy in Zhangjiagang,and to understand the prevalence of middle and late pregnant women in this area and its effect on pregnancy,and to provide some basis for perinatal health care in Zhangjiagang.[Methods]A total of 1225 pregnant women were enrolled in this study from January 2017 to June 2017.All pregnant women were enrolled in the middle of pregnancy(20-22 weeks of gestation)and late pregnancy(Pregnancy longer than 38 weeks or before delivery)at least two automatic blood analyzer for routine blood tests,and pregnant women in the middle of pregnancy using chemiluminescence microelectronic immunoassay for serum ferritin levels,tracking the situation of pregnant women until the end of childbirth.The treatment of pregnant women with iron deficiency anemia was analyzed to analyze the effect of serum protein levels during pregnancy on pregnancy outcome.This study excludes patients with cardiovascular disease,blood system diseases,endocrine system diseases,immune diseases,chronic kidney disease and prenatal bleeding.[Results]1.In patients with advanced pregnancy serum ferritin and iron deficiency anemia in general1.1 anemia in the middle and late pregnancyIn the middle of pregnancy: 219 cases of anemia,accounting for 17.88%(219/1225),a total of 121 patients with iron deficiency anemia,accounting for 55.25%(121/219);late pregnancy: 116 cases of new anemia,(78.76%),anemia patients with a total of 335 cases,accounting for 27.35%(335/1225),iron deficiency Anemia in 199 cases,accounting for 59.40%(199/335).1.2 treatment of patients with advanced anemia in pregnancyAmong 1225 patients,the mean serum ferritin level in the second trimester was(29.03 ± 30.11)ng / ml and in the second trimester there were 592 cases of iron deficiency(accounting for 48.33%(592/1225)).154 middle-stage anemia patients were treated(70.32%,154/219).78 cases of late pregnancy with iron deficiency anemia were cured after treatment,accounting for 67.24%(78/116)(P> 0.05)Comparison of parameters related to erythrocyte in patients with middle and late pregnancy.2.1 Comparison of mean erythrocyte volume(MCV)in middle and late pregnancy(P <0.05).The proportion of patients with small cell anemia in the middle of pregnancy was significantly lower than that in pregnant women(P <0.05).The proportion of patients with middle cerebral hemorrhage was significantly higher than that of pregnant women(P <0.05)The2.2 The average erythrocyte hemoglobin(MCH)and mean erythrocyte hemoglobin concentration(MCHC)distribution in middle and late pregnancy patients1 case(M> 0.05),MCHC increased in 2 cases of middle anemia and 1 case(M> 0.05)in patients with anemia in late pregnancy.MCH,MCHC normal,MCH and MCHC in patients with anemia were significantly different from those in patients with advanced pregnancy(P <0.05).2.3Correlation between Serum Ferritin Level and Anemia in Pregnant Women with PregnancyThe lower the serum ferritin,the higher the incidence of anemia,serum ferritin levels and mild anemia in the second trimester and the incidence of new anemia in the third trimester there is a linear relationship.The prevalence of serum anemia was significantly higher in pregnant women than in other groups(P <0.05).Serum ferritin 10-19.9 ng / ml had a significantly higher prevalence of anemia in the third trimester(P <0.05).[Conclusions]1.Zhangjiagang pregnant women in late pregnancy anemia rate of 27.35%,of which iron deficiency anemia rate of 16.24%,mid-pregnancy iron deficiency was 48.33%.Zhangjiagang area,although the economic level is high,but the prevalence of iron deficiency anemia is still high.2.The lower the level of serum ferritin in the second trimester,the higher the incidence of anemia in the second trimester,the higher the incidence of anemia in the third trimester.3.Increased serum ferritin combined with blood tests during pregnancy,the diagnosis and treatment of iron deficiency and iron deficiency during pregnancy has important practical significance.4.Pregnant need to be fully prepared before the pre-pregnancy physical examination,the younger,pre-pregnancy body mass index is too low,excessive weight gain during pregnancy too much pregnant women need to strengthen the mission and guidance.5.Pregnancy for pregnant women with iron deficiency and anemia to give active treatment to further promote maternal and child health.
