Application Of Resourcefulness In Patients With Nasopharyngeal Carcinoma In Symptom Distress And Benefit Finding

Objectives:To investigate the present situation of resourcefulness,benefit finding and symptom distress in patients with nasopharyngeal carcinoma,and analyze the influencing factors among the resourcefulness,benefit finding and symptom distress for patients with nasopharyngeal carcinoma,and also evaluate the effects of resourcefulness retraining on the resourcefulness,benefit finding and symptom distress of patients with nasopharyngeal carcinoma.It might provide the basic references for staff nurses on innovating the mode of health education.Methods:(1)A total of 304 patients with nasopharyngeal carcinoma were invited to finish the self-made questionnaire,Resourcefulness Scale,Benefit Finding Scale for Adults and The M.D.Anderson Symptom Inventory-Head&Neck inventory.The current status of resourcefulness,benefit finding and symptom distress of the patients with nasopharyngeal and their influencing factors were investigated and anylized.(2)On the basis of resourcefulness theory,the patients with radiotherapy of nasopharyngeal carcinoma in the First Affiliated Hospital of Nanhua University were selected as the subjects.The researcher chose the first ward of the First The Affiliated Hospital of University of South China as the control group,the second ward as the intervention group.If the number of cases is not enough,according to the randomized approach,the researcher numbered the new patients with nasopharyngeal carcinoma according to their order of admission and placing them into the controlling goup(n=36)and the retrainning group(n=36)by using the randomized approach.The demographic details,resourcefulness,benefit finding and symptom distress between the two groups were respectively compared.The patients in the control group were given routine health education,while the patients in intervention group were given resourcefulness training for intervention for 2 months other than the basic routine health education as in control group.Each group,the patients’ scores of the resourcefulness,benefit finding and symptom distress were assessed before or after the resourcefulness retraining.The data were analyzed using the Statistical Package for the Social Sciences(SPSS)18.0,the means,standard deviations and percentages were used as the statistical description;chi-square test,t-test,analysis of variance,multiple linear regression and linear correlation and other statistical methods for statistical analysis.Results:(1)The total average score of resourcefulness of the patients was(2.68±0.64),the average scores of two dimensions from the lowest to highest were social resourcefulness(2.62±0.71),personal resourcefulness(2.73±0.72).Age and educational level were the influencing factors for the patients’ resourcefulness.These two factors explained 27.3% of the the total variance.(2)The total average score of the patients’ benefit finding was(2.69±0.81).The average scores of six dimensions from the lowest to the highest were respectively as bellow: world view(2.25±0.90),personal growth(2.54±0.88),acceptance(2.65±0.94),health behaviors(2.79±1.16),family relations(3.18±1.30)and social relations(3.23±1.05).histological types,educational level and Age were the influencing factors for the patients’ benefit finding.These three factors explained 16.6% of the total variance.(3)The average score of the patients’ symptom distress dimensions was(3.96±1.47),the incidence was 100%;The highest five symptom distress score of symptom distress dimensions from the highest to the lowest were respectively as bellow: problem with tasting food(6.64±2.700),having a dry mouth(6.53±2.501),problem with mucus(6.35±2.949),lack of appetite(6.35±2.283),fatigue(5.10±2.359).The average score of the patients’ symptoms affect life dimensions was(3.62±1.81),the incidence was 98%;The dimensions scores of symptoms affect life dimensions from the highest to the lowest were respectively as following: enjoyment of life,mood,work(Including work around the house),relations with other people,general activity,walking.(4)The patients’ resourcefulness was positively correlated to the patients’ benefit finding(r=49.2%,P<0.01).(5)The patients’ first symptom distress was negatively correlated to the patients’ personal resourcefulness(r=-13.4%,P<0.05);the patients’ second symptom distress was negatively correlated to the patients’ personal resourcefulness(r=-21.8%,P<0.01)and was negatively correlated to the patients’ resourcefulness(r=-27.0%,P<0.01).(6)Before intervention,there were no significant differences for the demographic details,resourcefulness,benefit finding and symptom distress(P > 0.05).(7)After intervention,average scores of the patients’ resourcefulness,personal resourcefulness,social resourcefulness were more significantly reduced before intervention than those after intervention in the intervention group(P<0.05);average scores of the patients’ resourcefulness,personal resourcefulness,social resourcefulness were more significantly reduced in the control group than those in the intervention group(P<0.05).(8)After intervention,average scores of the patients’ benefit finding,acceptance,personal growth,social relations,and health behaviors were more significantly reduced before intervention than those after intervention in the intervention group(P<0.05);average scores of patients’ benefit finding,acceptance,family relations,personal growth,social relations and health behaviors were more significantly reduced in the control group than those in the intervention group(P<0.05).(9)After intervention,average scores of the patients’ symptom distress,symptom distress dimensions,symptoms affect life dimensions were more significantly reduced before intervention than those after intervention in the intervention group(P<0.05);average scores of the patients’ symptom distress,symptom distress dimensions,symptoms affect life dimensions were more significantly lowered in the intervention group than those in the control group(P<0.05).Conclusions:(1)The resourcefulness of the patients was at moderate level,age and educational level were the influencing factors for the patients’ resourcefulness.(2)The benefit finding of the patients was at moderate level,age,educational level and histological types were the influencing factors for the patients’ benefit finding.(3)The incidence of the patients’ first symptom distress was 100%;the incidence of the patients’ second symptom distress was 98%;the patients’ symptom distress has correlated with resourcefulness.(4)Resourcefulness retraining could enhanced benefit finding and palliated symptom distress.
