Objective: To explore the clinical efficacy and safety of venlafaxine in the treatment of epilepsy depressive disorder,And to explore the changes of amino acid metabolites and inflammatory factors in peripheral blood before and after treatment.Method: This study was a randomized controlled study,The study group and the control group of 56 cases.The study group received venlafaxine sustained release tablets treatment,The control group was given fluoxetine.Both groups were treated for 8 weeks.Respectively,at baseline,8 weeks late use(HAMD-17),the Barthel Index score and the Health Survey(SF-36 questionnaire)to assess clinical efficacy,The levels of serum DA,Hcy,5-HT and serum IL-2,IL-6 and TNF-? were measured at baseline and 8 weeks later.Results: 112 patients were enrolled in the group,Of which 106 patients completed the treatment(The study group and the control group were 3 cases due to sleepiness,nausea,dizziness,adverse reactions refused to continue treatment of shedding,Not included in the statistics).(1)At the end of the 8th week: Two groups of patients with HAMD-17 total score and block,cognitive impairment,sleep disorders,anxiety somatization of the factors were significantly lower than the baseline,The difference was statistically significant(P <0.05);The scores of HAMD-17 total score and block,cognitive impairment,sleep disorder and anxiety were lower in the study group than in the control group,The difference was statistically significant(P <0.05);(2)At the end of the 8th week: Barthel index scores were significantly higher in both groups than in baseline,The difference was statistically significant(P <0.05);The score ofBarthel index in the study group was higher than that in the control group,the difference was statistically significant(P <0.05).(3)At the end of the 8th week: The physical function,mental health,affective function,vitality,physical pain,physiological function,social function and overall health score of SF-36 questionnaire were significantly higher than those of baseline,The difference was statistically significant(P <0.05);The physical function,mental health,emotional function,vitality,physical pain,physiological function,social function and overall health score of the study group were higher than those of the control group(P <0.05).(4)At the end of the 8th week: Plasma DA and 5-HT levels in the two groups were significantly higher than those in the baseline,While the Hcy level was significantly reduced,The difference was statistically significant(P <0.05);The level of plasma DA and5-HT in the study group was higher than that in the control,while the Hcy level was lower than that in the control group,The difference was statistically significant(P <0.05);(5)At the end of the 8th week: The levels of serum IL-2,IL-6 and TNF-? in the two groups were significantly lower than those in the baseline group,The difference was statistically significant(P <0.05);The levels of serum IL-2,IL-6 and TNF-? in the study group were lower than those in the control group,The difference was statistically significant(P <0.05);(6)At the end of the 8th week,The total effective rate(88.68%)in the study group,Higher than the control group(83.02%),But the difference was not statistically significant(P> 0.05);(7)There was no significant difference in the incidence of adverse reactions between the two groups(P> 0.05).Conclusion: Venlafaxine treatment of epileptic seizures in patients with significant clinical efficacy,Can significantly improve the quality of life of patients,improve the ability of patients with daily activities,And less adverse reactions,good security;The levels of DA and 5-HT in peripheral blood were increased before and after treatment,Hcy levels were reduced,Serum IL-2,IL-6,TNF-? levels decreased,The change in the studygroup is more pronounced.Suggesting that these amino acid metabolites and inflammatory factors may be involved in the pathophysiological mechanism of epilepsy depressive disorder.
