Effects And Safety Of Laparoscopic Hepatectomy In Patients With Intrahepatic Bile Duct Stones

Objective:To evaluate the clinical effects and safety of laparoscopic treatment in patients with intrahepatic bile duct stones.Methods:A total of 78 cases with intrahepatic bile duct stones from January 2013 to February 2017 in our hospital were include in this study.Clinical data were retrospectively analyzed.All csaes were divided into two groups according to the operative methods,the Laparoscopic Group(LG)and the Opening Group(OG).Cases of LG underwent laparoscopic surgery and those of OG underwent opening surgery.There were no statistical difference between the two groups in the general data(P>0.05).The related parameters such as operative duration,blood loss,time of hospital stay,operative complications and so on were analyzed.Results:The time of operation and inflow occlusion in LG and OG were 211.8±65.1min vs 178.5±77.2min,43.9±22.6min vs 21.8±15.8min,respectively.The data in LG were significantly higher than those in OG(P<0.05).The blood loss,recovery eating time,leaving bed time,hospital stay,time of abdominal drainage,cases with analgesia pump,SIRS in LG and OG were 123.8±79.9mlvs 198±88.4ml,0.9±0.6d vs 1.1 ±0.5d,1.2±0.5d vs 2.5±0.8d,7.8±4.3d vs 10.3±5.5d,3.3±2.6d vs 5.8±3.4d,5cases vs 32cases,2cases vs 7cases,respectively.The data in LG were significantly less than those in OG(P<0.05).There was no incisional infection case in LG,but 4 cases in OG(P<0.05).As for the postoperative complications,such as hemorrhage,intra-abdominal infection,bile leakage,residual stones,operative death,there were no remarkable differences between LG and OG(P>0.05).There were 10 cases with T tube placement in LG and 8 cases in OG.There was no remarkable significance between two groups(P>0.05).Conclusion:Laparoscopic hepatectomy for the treatment of patients with intrahepatic bile duct stones is safe and effective.
