1 ObjectiveThe study aims at searching methods that are applicable in the research of TCM syndrome types of Henoch-Sch?nlein purpura through the application of metabonomics technology and analytic methods,which is based on the guideline of Henoch-Sch?nlein purpura and it's syndrome differentiation scale of TCM.The other purpose of the study is to seek out the different serum metabolites and biomarkers of Henoch-Sch?nlein purpura and it's commonly syndromes of TCM,and give reference to the study of material basis of TCM syndromes.The subject research is regard as science and technology key project of henan province.(ITEM NUMBER:172102310061)2 Methods(1)Collection of cases and the sectionalization:Collected cases in Pediatric clinic and the ward of our hospital,selected children with Henoch–Sch?nlein purpura that met the standards(n=60),include purpura with syndrome of wind-heat injuring collaterals group(n=30)and purpura with bleeding due to blood-heat group(n=30).Collected their blood specimens,and 10 healthy children as control group.(2)Mass spectrometry detection:Observed all specimens used UPLC-QTOF/MS after the pretreatment,obtained all compounds' data information.(3)Identification and screening of differences metabolites: Imported all data to the relevant software to identify compounds and compared the quantitative results between the two groups.Used multivariate statistical and model identification methods to screen out the different metabolites between Henoch–Sch?nlein purpura group VS the control group and purpura with syndrome of wind-heat injuring collaterals group VS purpura with bleeding due to blood-heat group.Determined the related biomarkers preliminary by searching in HMDB database and KEGG database,then analyzed their metabolic pathways.3 Results(1)In mode discrimination diagrams of PLS-DA and OPLS-DA,samples which are belong to Henoch–Sch?nlein purpura group and the control group are aggregating distribution separately.Samples which are belong to Henoch–Sch?nlein purpura group are distributed on the left side of center vertical line,while samples which are belong to the control group are distributed on the other side.It states that the metabolic status between Henoch–Sch?nlein purpura group and the control group have significant differences.(2)In mode discrimination diagrams of PLS-DA and OPLS-DA,samples which are belong to purpura with syndrome of wind-heat injuring collaterals and purpura with bleeding due to blood-heat are aggregating distribution separately.Samples which are belong to purpura with syndrome of wind-heat injuring collaterals are distributed on the left side of center vertical line,while samples which are belong to purpura with bleeding due to blood-heat are distributed on the other side.It states that the metabolic status between purpura with syndrome of wind-heat injuring collaterals and purpura with bleeding due to blood-heat have significant differences.(3)After the comparative study of Henoch–Sch?nlein purpura group and the control group,it shows that there are 7 different metabolites between them:ADP-ribose,2'-phosphate,Deoxyadenosine-triphosphate,N-Acetylleucine,Citicoline,8'-Hydroxyguanosine,nositol-1,3,4,5-tetraphosphate,Iodotyrosine.Except Iodotyrosine,other metabolites all have more content in Henoch–Sch?nlein purpura group than the control group.They are related to Purine metabolism,Tyrosine metabolism,Glycerophospholipid metabolis and Inositol phosphate metabolism.(4)After the comparative study of purpura with syndrome of wind-heat injuring collaterals and purpura with bleeding due to blood-heat,it shows that there are 6different metabolites between them:Erythronic acid,Phenylalanylphenylalanine,8-Hydroxyguanosine,Phytosphingosine,Deoxyadenosine triphosphate and N-Acetylleucine,all metabolites have more content in purpura with bleeding due to blood-heat than purpura with syndrome of wind-heat injuring collaterals.They are related to Sphingolipid metabolism and Purine metabolism.(5)The result of the study shows that Henoch–Sch?nlein purpura group VS the control group and purpura with syndrome of wind-heat injuring collaterals group VS purpura with bleeding due to blood-heat group have 3 common different metabolites:Deoxyadenosine triphosphate,N-Acetylleucine and 8'-Hydroxyguanosine.4 Conclusion(1)Compared purpura with syndrome of wind-heat injuring collaterals and purpura with bleeding due to blood-heat,they have 6 different metabolites:Erythronic acid,Phenylalanylphenylalanine,8-Hydroxyguanosine,Phytosphingosine,Deoxyadenos ine triphosphate and N-Acetylleucine,related to 2 metabolic pathways:phingolipid metabolism and Purine metabolism.It can give reference to the study on material basis of TCM syndromes.(2)Metabonomics has significant advantages on material basis research about human body's physiological and pathological state.It has a broad space for development.
