Objective:To explore the correlations about the grades?grade??????and different molecular subtypes of breast cancer with the parameters based on the intravoxel incoherent motion?standard apparent diffusion coefficient?slow ADC-mono?fast ADC-mono?fraction of fast ADC-mono?.Materials and Methods:Sixty-six patients were retrospectively collected and confirmed that have suffered from invase ductal carcinoma of breast with histological grade and immunohistochemical analysis results. The histological grade?????were divided into low-grade group,middle-grade group and high-grade group, respectively.According to the immunohistochemical analysis results, four different molecular subtypes of breast cancer were generated. All patients had underwent preoperative breast MRI with intravoxel incoherent motion diffusion-weighted imaging in our institution. All checks were performed on 3.0T MRI scanner?GE,medical systems,American?.IVIM parameters(Standard ADC?Dmono?D*monoono and Fmono)were obtained by AW4.6 workstation,All the different parameters were analyzed by one-way ANOVA and then use the Spearman rank correlation test to analyze the correlation about the grading of breast cancer with the parameters.Through making receiver operating characteristic?ROC?curves, analyze the diagnostic efficient of different parameters. And then use kruskal-wallis test to analyse that do the parameters have differences in the different molecular subtypes of breast cancer?Result:All patients underwent surgical resection treatment. The pathological findings were all invasive ductal carcinoma and a total of 66 lesions were found,of which 14cases were in grade?,43 cases in grade?and 9 cases in grade?.According to the immunohistochemical analysis results, there are 14 cases Luminal A breast cancer,32cases Luminal B breast cancer,9 cases Triple negative breast cancer,11 cases HER-2overexpressed breast cancer. The standard ADC value and the parameters Dmonoono value of the lesion were negatively correlation with the histolological grade of breast cancer?P<0.05?,the correlation coefficients were-0.400 and-0.489,respectively?While the parameter of D*monoono and Fmonoono value had no statistically significant differences between the different grade?P>05?.The standard ADC and Dmonoono value were analyzed by ROC curves.when the cut-off values?were 0.574 and 0.621,respectively,The sensitivity?specificity and the area under the ROC curve of the standard ADC and Dmonoono values were 78.6%and 78.8%,0.807,92.9%,69.2%,0.847.Further analysis found that the parameters of standard ADC value and Dmonoono had statistically significant differences in different molecular subtypes of breast cancer. But the parameters of D*monoono and Fmonoono value had no statistically significant differences in different molecular subtypes of breast cancer. Conclusion:Parameter of standard ADC value and Dmonoono value based on the intravoxel incoherent motion can be use to predict the grading of breast cancer. In different molecular subtypes of breast cancer, the parameters of standard ADC and Dmonoono value had statistically significant differences,it may provide the basis for the choice of breast cancer treatment.
