Objective: To investigate the clinical effect of different doses of metoprolol in the treatment of chronic heart failure.Methods: 56 cases of chronic heart failure were selected as control group,50 patients admitted to hospital in the same period as the experimental group.All the patients were given cardiac stimulant,diuretic and vasodilators and other conventional drug treatment.The control group was given a small dose of metoprolol,initial dose of 6.25 ~12.5 mg/d,at a rate of 25 ~ 75 mg/d,the maximum dose was 75 mg/d.The experimental group were given large doses of metoprolol in the dose of more than 50mg/d,the weekly increase of 25 mg,a maintenance dose of 100~150mg,the maximum dose is less than 150mg/d.Clinical effects and Blood pressure were compared Between the two groups Before treatment and 12 weeks after treatment,doppler ultrasonography in detecting left ventricular ejection fraction,left ventricular end diastolic and end systolic volume.Results: 1.Thecardiac function classification(NYHA classification)were improved significantly after the treatment in both groups.6 minutes walking distances were also increased significantly.Systolic and diastolic blood pressure as well as NT-pro BNP were decreased significantly.2.The improvement of left ventricular ejection fraction,left ventricular end diastolic and end systolic volume in the experimental group were better than those in the control group(P<0.05).3.The NYHA class,6-minute walking distance and NT-pro BNP were significantly improved in the experimental group than that in the control group(P<0.05).4.Two groups of different doses of metoprolol were well tolerated,with no serious complications or discontinuation of treatment.Conclusions: Higher dose of metoprolol can improve heart function and has better therapeutic safety for chronic congestive heart failure.
