BackgroundCytokine inflammatory immune response participates in the pathological process of coronary sclerosis.IL-27 belongs to a new member of the IL-6/IL-12 family and participates in the process of myocardial disease,but its expression level in different degrees of coronary heart disease(CHD),as well as the relationship with traditional risk factors for coronary heart disease,have not been reported.ObjectiveBy observing and testing the serum IL-27 levels and clinical characteristics of normal and different grades coronary heart disease,analyzing its association with coronary heart disease.MethodsAll patients were divided into four groups according to the results of coronary angiography,including stable angina pectoris(SA),unstable angina pectoris(UA),acute myocardial infarction(AMI)and control group(CO).Determination of serum IL-27 levels in all patients.According to a unified questionnaire to collect clinical relevant data,using case-control multivariate Logistic Regression analysis method to analyze its association with coronary heart disease risk.ResultsThe serum concentration of IL-27 in the 4 groups was:stable angina pectoris(SA)group(58 cases):286.69±104.27 pg/ml,unstable angina pectoris(UA)group(114 cases):417.35±181.23 pg/ml,acute myocardial infarction(AMI)group(70 cases):505.34±193.88 pg/ml and control group(CO)group(41 cases):181.21±95.23 pg/ml.The serum il-27 levels in three disease groups were all higher than control group.Compared with control group,the expression level of IL-27 in patients with SA,UA,AMI were statistically different(P values were 0.004,<0.001,<0.001);In the U A group and the AMI group,the serum IL-27 level in the AMI group was increased,but P>0.05.There was no significant difference between the two groups.Correlation analysis revealed that statistically significant factors related to serum IL-27 level include gender,history of diabetes,creatine kinase isoenzyme-MB(CK-MB),cardiac troponin I(CTNI),triglyceride(TG),and high-density lipoprotein(HDL).Multivariate logistic regression analysis of gender,hypertension history,diabetes history,smoking,HDL,TG,CK-MB,and CTNI showed that the serum IL-27 level was comparable to that of the control group and the three disease groups.The correlation degree reached a statistically significant level,and the relative risk(OR)and 95%confidence interval values were:OR = 1.008 in the SA group,95%CI=1.003-1.014,OR = 1.017 in the UA group,95%CI = 1.011-1.0239 and AMI group OR = 1.021,95%CI = 1.014-1.027.At the same time,it is seen that hypertension increases the risk of unstable angina and acute myocardial infarction.ConclusionSerum high levels of IL-27 increase the risk of CHD and Acute myocardial infarction.This study also confirmed that hypertension is a risk factor for UA and Acute myocardial infarction,both are independent risk factor of coronary heart disease.
