BackgroundWith the aging of population in China,ischemic cerebrovascular disease is on the rise.Stroke in China is the first place in the world.Early detection of cerebral ischemic foci is one of the important means to save patients' life and improve their quality of life.With the development of cerebral perfusion technique,Pseudo-continuous arterial spin labeling(PCASL)is a new technique developed on the basis of arterial spin labeling(arterial,spin,labeling,ASL).More and more clinical attention.The combined application of pseudo continuous arterial spin labeling(PCASL)and dynamic susceptibility contrast enhanced perfusion imaging(dynamic susceptibility contrast enhanced DSC)technology,to explore the application value of PCASL in ischemic cerebrovascular disease.ObjectiveTo evaluate the value of pseudo continuous arterial spin labeling perfusion imaging(PCASL)and dynamic contrast enhanced magnetic perfusion imaging(DSC)in the diagnosis of ischemic cerebrovascular disease.MethodsRoutine MRI,DWI,ASL and DCS-PWI scans were performed in 37 patients with clinically diagnosed cerebral infarction,refer to the DWI image,infarct size(region of interest,ROI)CBF value and CBF value of contralateral mirror site were measured on PCASL and DSC respectively.The location,size,extent,and ischemic penumbra(IP)of the perfusion abnormalities were recorded.Results37 cases of cerebral infarction,Clinical follow-up confirmed 26 cases of cerebralinfarction,TIA 7 cases.In 26 cases of cerebral infarction,21 cases were consistent with perfusion of two methods(21 cases with low perfusion,5 cases with high perfusion),and both of them were tested by kappa,and the agreement was good.In 21 cases of low perfusion,the PCASL-CBF values of 19 cases were lower than the DSC-CBF value,and the difference was statistically significant by paired data t test(P < 0.05).7 cases of TIA,6cases were consistent with perfusion(6 cases with low perfusion,1 cases with high perfusion),.ConclusionsPCASL and DSC showed a high consistency in perfusion status of infarct lesions,PCASL were more sensitive than DSC in showing hypoperfusion,Combination of DWI with ischemic penumbra is of clinical value,and ASL is helpful for the early diagnosis of TIA.
