A Study On Operation Efficiency Of Township Health Centers In Jiangxi Province Based On Data Envelopment Analysis(DEA)

Objective:The purpose of this study is to evaluate the operation efficiency of the township hospitals in Jiangxi from 2012 to 2014,understanding whether the input and output structure of the township hospitals is reasonable and discussing what aspects can be used to improve the medical conditions and service levels of the township hospitals.Provide scientific basis for the government to promote health care reform in township hospitals.Method:1.Describe the basic situation of the staff of township hospitals in Jiangxi Province,the construction and utilization of health infrastructure,and the economic benefits.2.Use the cluster analysis method to select representative input and output indicators of township hospitals in Jiangxi Province.3.Use data envelopment analysis(DEA)to analyze the overall efficiency,pure technical efficiency,and scale efficiency of township hospitals.4.DEA-Malmquist method analyzes the dynamic changes of total factor productivity of township hospitals from 2012 to 2014.Results:1.The total number of workers,number of health technicians,number of practicing physicians,and number of nurses in township and township hospitals in Jiangxi increased continuously from 2012 to 2014.The total number of employees increased by 2.08% on average annually.And the average annual increase in the number of health technicians,the speed is 1.15%.The ratio of health care between 2012 and 2014 remained basically unchanged at 1:0.61.The overall age distribution of health personnel in township hospitals in Jiangxi Province was distributed in a football-like manner.The 25-44-year-old population was the main body,and there was a tendency for young talents to grow slowly.2.In terms of quantity,the actual number of beds,building construction area,and the number of equipment above RMB 10,000 have increased year by year between 2012 and 2014.Among them,the average growth rate of the number of equipment above 10,000 yuan is the fastest.3.The number of patients undergoing general treatment and treatment increases year by year,and the number of discharged people appears to increase first and then decrease.The utilization rate of beds is low,and the utilization rate of beds is declining year by year.4.The total income of township hospitals in Jiangxi Province from 2012 to 2014 increased from 4.0338 million yuan to 4.781 million yuan,an increase of 18.53% in 2014 compared to 2012,with an average annual growth rate of 9.08%.The total expenditure for 2012-2014 increased from RMB 3,428,900 to RMB 4,304,400,an increase of 25.71% in 2014 compared with 2012,and the average annual growth rate was 12.25%,which was higher than the growth rate of total income.From 2012 to 2014,the overall economy of township health centers in Jiangxi Province grew steadily.The balance of revenue and expenditure was basically balanced with a slight surplus,but the balance of income and expenditure decreased year by year.5.The average scale efficiency of township and township hospitals in various regions in 2012-2014 was 0.976,0.897,and 0.860,respectively,which were all close to 1.Between different years,there are invalid areas of scale.In the decision-making unit that has not reached the optimal size,except for Jingdezhen and Yingtan,the scale is increasing,and the rest are decreasing returns to scale.6.The average total factor productivity change index(TFPch)for the input and output of township hospitals from 2012 to 2014 was 1.019,the annual average value of the technical change index was 1.143,the annual average value of the efficiency change index was 0.892,and the total factor for 2012-2014 was 6.The main reason for the increase in productivity is the increase in the index of technological change.7.Combined with DEA efficiency analysis and relaxation variables,in 2012,there were eight districts and township health centers with effective DEA,and their slack variables were also corresponding to zero.Jiujiang region needs to reduce total expenditures and utility beds,and Ganzhou region needs to reduce total expenditures.The Shangrao region needs to reduce total expenditure and total assets to achieve the best efficiency of medical service output.In 2013 and 2014,the number of township and township hospitals as a whole was less than that in 2012,indicating that the resources allocated to the current three-year allocation of health resources have not been fully utilized and have not reached the optimal scale.Therefore,the adjustment of township hospitals needs to be adjusted.The scale of services to improve the efficiency of medical services.Conclusion:1.The total number of health personnel in township hospitals in Jiangxi Province has increased year by year,but there are few medical service personnel.The distribution of personnel structure varies greatly among different regions.The proportion of young health technicians is relatively small,and the qualifications of health technicians constitute a single unit.Compared with the national average,there is still a big gap between the ratio of health care for township health centers.2.According to the DEA efficiency analysis,the efficiency of medical services in township and township hospitals in some areas is relatively non-DEA-effective,mainly because of lack of investment in infrastructure,low utilization of available health resources,and lack of scientifically feasible management models..3.The income and expenditure of township hospitals in 2012-2014 increased year by year,and the proportion of financial subsidies continued to increase,but the efficiency of medical services did not increase in the same proportion.Therefore,blindly increasing the input of health resources will not improve the efficiency of medical services in township hospitals.It will not only increase the financial burden on national governments,but also may result in low performance of inputs.The key to solving these problems is not simply to increase government investment.It is to increase its operating efficiency.4.The results of the study of variable variables analysis show that the main reason for the relative ineffectiveness of DEA in some township health centers is that the total expenditure of medical institutions,the total number of employees,and the number of beds are low,while the utilization rate is low.The number of visits to hospitals and bed occupancy rates are The main reason for DEA’s ineffective output.Therefore,according to local conditions,the establishment of a reasonable management plan is essential for the healthy development of township hospitals.
