Study On The Efficacy And Safety Of Radiofrequency Ablation Of Canine Renal Artery Adventitia Sympathetic Nerve

?Objective?: To verify the efficacy and safety of radiofrequency ablation of sympathetic renal artery in dogs.To explore the advantages and disadvantages of using the new Flashpiont TM multipolar radiofrequency catheter ablation in renal sympathetic nerve ablation.?Methods?:FlashpiontTM renal artery multipolar radiofrequency ablation catheter tip for spiral curved shape,with four ablation electrode 14 healthy dogs(weight 20-30kg)were selected,and the blood pressure was measured by one side femoral artery puncture and catheterization,and the blood samples were taken from the femoral vein.Then randomly divided into 2 groups,one group of femoral artery sheath FlashpiontTM renal artery ablation catheter into unilateral renal artery discharge for endometrial ablation,ablation group,ablation parameters reference Symplicity series of experimental parameters(the experimental setting is: temperature 40,energy 10 watts,ablation time of 60 seconds);group open separation of renal artery FlashpiontTM after renal artery ablation catheter in unilateral renal artery wound discharge ablation for epicardial ablation group,ablation parameters with endometrial ablation group.Respectively after 7 days,30 days,90 days of renal artery angiography and renal artery blood review,organized and executed in dogs after pathological examination Comparing the canine artery blood pressure,blood NE,Cr,BUN,Na have no difference;postoperative renal artery sympathetic nerve pathological examination,observation of renal artery and sympathetic nerve ablation effect and complications? Results ? : 14 dogs in the experiment,1 rats died after anesthesia(suspected respiratory depression),1 rats died during laparotomy(suspected hemorrhagic shock),2rats died 24 hours after operation(unknown cause),more than 9 dogs were successfully completed surgery(endometrial ablation group 5,epicardial ablation group 4)during the operation,no renal artery dissection,occlusion,perforation and other complications.The longest survival time was 67 days(adventitia ablation group).No renal artery thrombosis or renal artery stenosis was found after operation.Two groups of operation time was significantly shortened with the accumulation of experience.Postoperative blood pressure decreased,but there was no significant difference between the two groups(P>0.05);theactivity of sympathetic nerve(NE)decreased,but there was no significant difference between the two groups(P>0.05).Postoperative renal artery diameter loss,epicardial ablation group diameter(P<0.05),lose less epicardial ablation effect on renal artery diameter smaller Postoperative renal function(serum Cr,BUN)is damaged,the damaged degree of epicardial ablation group lighter(P<0.05)The blood Na was decreased after operation,but there was no significant difference between the two groups(P>0.05).Pathological observation showed that the sympathetic nerve of the renal artery was obviously necrosis after radiofrequency ablation,and the renal artery was repaired in different degrees with the passage of time.?Conclusion?:Radiofrequency ablation of renal artery adventitia sympathetic nerve renal artery adventitia sympathetic nerve is a safe and effective procedure.Flashpiont TM renal artery multipolar radiofrequency ablation catheter of the multi electrode spiral design is reasonable,adherence to good effect,can realize multi quadrant ablation,so that the operation process is simplified,more effectively damaged sympathetic nerve ablation to achieve.
