Meta Analysis Of The Value Of Von Willebrand Factor In The Diagnosis Of Portal Hypertension In Liver Cirrhosis

Objective A meta-analysis was used to investigate the value of serum vWF in the diagnosis of portal hypertension in liver cirrhosis.Methods The computer searches articles published in Cochrane Libary?Pubmed?CNKI?EMBASE and Wanfang database.According to the inclusion and exclusion criteria independently screened literature,extracted data from the literature,the final used QUADAS can be evaluation of the methodological quality in the studies included.First,use Meta software to analyze the sensitivity and specificity and 95%CI of vWF in the diagnosis of portal hypertension in liver cirrhosis,then Meta-DiSc1.4 software which can be used to analyze sensitivity(SEN),specificity(SPE),positive likelihood ratio(+LR),negative likelihood ratio(LR),diagnostic odds ratio(DOR)combined and test for heterogeneity.At the end,that can be used to draw summary receiver operating characteristic(SROC)curve,calculate the area under the curve(AUC).Results The final four references were included,a total of 662 cases of the research object,the analytic of the results through the Meta-DiSc1.4 software showed that SEN combined =0.823 [95%CI(0.788,0.855)],SPE combined =0.782,[95%CI(0.708,0.845)],+LR combined =3.777,[ 95%CI(2.794,5.107)],-LR combined =0.221[95%CI(0.180,0.272)],DOR combined =18.347[ 95%CI(11.725,28.708)],AUC =0.8896.Conclusion Serum vWF is closely related to the occurrence and development of portal hypertension in liver cirrhosis,and the expression of the patients with portal hypertension in liver cirrhosis is significantly increased.The serum vWF has a good effect on the diagnosis of liver cirrhosis with portal hypertension.It can be used as an effective and feasible method for noninvasive prediction of portal hypertension in liver cirrhosis.However,further studies are needed to evaluate the severity of liver cirrhosis with portal hypertension.
