Objective:Comparative analysis of imaging features of alveolar echinococcosis and tuberculosis of vertebral body.Methods:CT and MR imaging features of alveolar echinococcosis of spine were retrospectively analyzed by reviewing the 20 cases confirmed by clinic and radiology.Retrospectively analyzed 30 cases of spinal tuberculosis confirmed by pathology in our hospital from in recent years.Of 20 cases of alveolar echinococcosis of spine,8cases were scanned by CT and 3D reconstruction,2 cases were scanned by MRI,10cases were scanned by both CT and MR?6 cases performed enhanced spiral CT scan and 4 cases performed enhanced MR scan?.30 cases were all scanned by CT and3D reconstruction,13 cases were scanned by MRI.Analyze bone destruction,corresponding vertebral accessory,adjacent ribs,disc space,mass of paravertebral muscles and calcification of lesions.The data was tested by using?2 test.Results:20 cases of alveolar echinococcosis of patient which primary lesions are from the liver,three vertebral bodies were involved in 12 cases,two adjacent vertebral bodies were involved in 4 cases,single vertebral body was involved in 4 cases,corresponding vertebral accessory was involved in 18 cases,adjacent ribs were involved in 12 cases,irregular mass was formed in paravertebral muscles with ill-defined margins in 18 cases,disc space was narrowed in 6 cases.There were 10 cases of pulmonary metastasis and 12 cases of retroperitoneal metastasis.On CT scans,the lesions of vertebral body showed osteolytic and geographical bone destruction with bone sclerosis.The lesions of vertebral accessory showed swelling and osteolytic bone destruction,small cystic density and calcification of cystic wall and irregular calcification were found in 14 cases.On MR scans,the lesions of vertebral body and paravertebral muscles showed high and low mixed signal,with hypointensity on T1WI,hypointensity on T2WI,vertebral accessory and adjacent ribs were involved in 12 cases.Small cyst with T2hyperintensity was found in 8 cases.30 cases of spinal tuberculosis,three vertebral bodies were involved in 6 cases,two adjacent vertebral bodies were involved in 24cases,corresponding vertebral accessory was involved in 7 cases,adjacent ribs were involved in 5 cases,disc space was narrowed in 24 cases,the lesions of vertebral body showed osteolytic bone destruction with bone sclerosis,fusiform mass was formed in paravertebral muscles with defined margins in all cases,irregular calcification were found in 6 cases.between alveolar echinococcosis cases and tuberculosis cases of vertebral body were found in the corresponding vertebral accessory,adjacent rib,disc space,small cystic density and calcification are significant statistical differences?P<0.05?.Conclusion:Alveolar echinococcosis is a regional disease,imaging features of small cystic density or cystic signal and arc calcification in mass,and corresponding vertebral accessory usually were involved and disc space was seldomly narrowed are helpful for the diagnosis of and differential diagnosis of these two diseases.
