| Objective:Platelet is an important ingredient in blood,which plays a very pivotal role in coagulation mechanism.The deduce of platelet always leads to hemorrhagic tendency.Thus,most studies put emphasis on the risk of postpartum hemorrhage.Meanwhile,the hypercoagulability is also a risk factor of thrombosis when women are pregnant.However,women’s living in oxygen-deficient environment in Qinghai province is the other factor of hypercoagulability.Anoxic atmosphere makes blood prone to be hypercoagulable,in the same time,activate platelet leads to thrombosis.Because of both oxygen-deficient environment and hemodynamic change may lead hypercoagulability and thrombocytopenia during pregnancy.The relationship between them is considerable.Gravidas in Qinghai province are suffering from both thrombocytopenia and hypercoagulability may occur increasingly during her pregnancy,and lead to postpartum hemorrhage or thrombophilia.This study focused on the difference between inpatient in Qinghai province who were diagnosed as thrombocytopenia during pregnancy for the first time(observation group)and those who had normal platelet(control group),our purpose was to determine the risk of thrombophilia and postpartum hemorrhage,in addition the difference between gestational age、delivery method、neonatal weight between two groups have been compared to analyze pregnancy outcome.Methods:We collected 101 gravidas who delivered in Qinghai Provincial People’s Hospital from August 2016 to December 2017 who were diagnosed as thrombocytopenia.We also collected 151 gravidas who have normal platelet at the same time span.To evaluate the risk of thrombophilia,PLT,HCT,PT,APTT,TT、Fbg and D-dimer down had been investigated.The correlation between hypercoagulability and thrombocytopenia had also been concurrently investigated.The difference of the value of hemoglobin,antenatal and postpartum blood loss of gestational age,delivery method and neonatal weight had been compared between two groups to analyze pregnancy outcome.Results:1.Platelet in observation group was significantly negative correlatived with HCT、Hb(P<0.05);was absence of correlation with TT、PT、APTT、FIB、D-dimer(P>0.05).In control group,platelet was absence of correlation with HGB、HCT、TT、PT、APTT、FIB D-dimer(P>0.05).2.The difference value of hemoglobin and HCT had no difference between two groups(P>0.05).in observation group,when Platelet>70×10~9/L.3.HCT、PT、APTT、TT、FIB、D–dimer between two groups had no difference(P>0.05),APTT between two groups had statistic difference between two groups(P<0.05).4.gestational age、delivery method、neonatal weight between two groups had no difference(P>0.05).Conclusion:1.The standare of thrombocytopenia in Qing Hai province was≧50×10~9/L.2.Thrombocytopenia was related to HCT and Hb.Both HCTand Hb had been increased,but the platelet had been reduced.3.Thrombocytopenia did not affect the pregnancy outcome in Qing Hai province.4.When platelet>70×10~9/L,the blood loss would not be increased.5.Thrombocytopenia did not affect gestational age、delivery method、neonatal weight. |
