Objective To analyze the the correlation between imaging distribution and vascular lesion site in posterior circulation infarction. Methods Patients with posterior circulation infarction were enrolled in the study. The sites of the infarcts were divided into proximal, middle, distal andmixed group infarctions according to the results of MRI.The vertebrobasilar artery were divided into the extracranial segment of the vertebral artery, the intracranial segment of the vertebral artery, the basilar artery, and the posterior cerebral artery according to the results of vascular examination. The correlation between the distribution of posterior circulation infarcts and the sites of vertebrobasilar system lesions was analyzed. Results A total of 227 patients with posterior circulation infarction were enrolled. There were 52 cases(22.91%)in the proximal group,87 cases(38.33%)in the middle segment,62 cases(27.32%)in the distal group, and 26 cases in the mixed group(11.45%).There were 90 cases(39.65%)of vertebral basilar artery disease,of which 31 cases(13.66%)in the proximal group,33 cases(14.54%)in the middle group,12 in the distal group(5.29%),and 14 cases(6.17%)in the mixed group. In different groups, the rate of vascular lesions was different(P=0.000),and the incidence of vascular lesions in the near segment was higher than that in the middle group(P=0.013)and the distant group(P=0.000),higher than the mixed group(P=0.627).In the posterior circulation cerebral infarction, the lesion vessels in each group were mostly seen in the vertebral artery. In the case of vertebral artery disease, the intracranial lesion rate of vertebral artery was not higher than that of the extracranial segment(P=0.965).In the distal group, the basal artery of the mixed group and its branchlesions were more seen in the posterior cerebral artery, the difference was statistically significant(P=0.015).Conclusion 1.The distribution of posterior circulation cerebral infarction is more common in the middle and distal segments.2.Posterior circulation cerebral infarction is closely related to vertebral artery disease(stenosis ? 50%).However,intracranial and extracranial lesions of the vertebral artery are absent difference.3.In the four-group analysis, the proximal infarction was closely related to vertebral artery lesions(stenosis ?50%).4.Infarction in the proximal and middle segments is more common in the basilar artery.while the distal and mixed segmental infarctions are more common in the posterior cerebral artery.
