| Objective: To investigate the clinical features,immunology,laboratory tests and imaging features of psoriatic arthritis.Methods: The clinical data of 215 psoriatic arthritis patients admitted to the General Hospital of Ningxia Medical University from April 2007 to April 2017 are recorded,including general information,clinical manifestations,comorbidities,serum,immunological indexes and related imaging data.The clinical characteristics of male and female,Hui and Han nationality are analyzed retrospectively,and the clinical characteristics of PsA patients are analyzed according to whether the HLA-B27 or ANA is positive or not.Results:(1)Among 215 patients,the number of male is 118,and the female is97.The ratio of male to female is 1.22:1,the onset age is from birth to 68 years old,the average age of onset is 31.5±12.73 years old,the course is from 3 to 480 months,and the average course of disease is 154.01±122.47 months.26 patients(12.1%)have a family history of psoriasis.The skin lesions manifested prior to arthritis is 171 cases(79.60%),arthritis prior to skin lesions occurred is 22 cases(10.20%),and both symptoms of skin and joints appeared simultaneously is 22 cases(10.20%).Psoriasis vulgaris is the most common type.As for the types of arthritis,73 cases(34%)are asymmetrical oligoarthritis,60 cases(27.9%)are symmetrical polyartritis,14 cases(6.5%)are distal interphalangeal arthritis,6 cases(2.8%)cases are mutilation patterns,and 50 cases(23.3%)are spondylitis.The most common affected joints are proximal interphalangeal joints in 103 cases(47.9%).Systemic symptoms can occur fever in 17cases(7.9%),morning stiffness in 35 cases(16.3%),and PsA combined with lung damage in 28 cases(13%),cardiac damage in 34 cases(34.3%),blood system damage in 78 cases(36.3%),ocular damage in 6 cases(2.8%),kidney damage in 27 cases(12.5%),metabolic disturbance in 84 cases(39.1%).The imaging of sacroiliac joint is susceptible to HLA-B27.(2)Female patients prone to multiple joints invasion,and male patients prone to involvement in the axial joints,and the positive rate of HLA-B27 is higher in male patients.(3)Symmetrical polyartritis is the more common type in ANA positive patients(P<0.05).(4)The onset age of HLA-B27 positive patients is earlier(P<0.05),and sacroiliac joint involvement is more common(P<0.05).(5)The incidence of metacarpophalangeal joints,the proximal interphalangeal and distal interphalangeal joints of the toes is higher in Han patients(P<0.05).Conclusions:(1)Psoriatic arthritis is common in men,and male patients are more likely to involve the axial joints,the positive rate of HLA-B27 is higher.(2)Psoriasis vulgaris,asymmetric oligoarticulation and proximal interphalangeal joint involvement is the most common.(3)PsA patients may be combined with impairment of the heart,lungs,eyes,kidney,blood system and easy to appear metabolic dysfunction.(4)ANA positive patients are more likely to suffer from multiple joints.The onset age of HLA-B27 positive patients is earlier,and sacroiliac joint involvement is more common.(5)The susceptibility of metacarpophalangeal joints,the proximal interphalangeal and distal interphalangeal joints of the toes is higher in Han patients. |
