Application Of ~1H-MRS Combined With DWI In Grading Diagnosis Of Gliomas

Objective:The ratio of related metabolites in ~1H-MRS and the apparent diffusioncoefficient(ADC),relative diffusivity coefficient(rADC)in DWI were evaluated.And the application value in Grading Diagnosis of Gliomas.Methods:~1H-MRS and DWI imaging data of suspected gliomas were prospectivelycollected.According to the postoperative pathology,the patients were divided into two groups according to WHO(2016)criteria:low grade glioma group(I~II grade,26 cases)and high grade glioma group(III~IV grade,30 cases).The Cho/NAA,Cho/Cr,NAA/Cr,ADC value and rADC value in the two groups were compared and analyzed statistically.Results:The ratios of Cho/Cr,Cho/NAA in high grade glioma group(3.58±2.04,3.93±2.47)were significantly higher than those in low grade glioma group(2.13±0.59,2.14±0.66)(all P<0.05).NAA/Cr ratio,ADC value and r ADC value in high grade glioma group(0.54±0.32,1.14±0.25,1.46±0.32)were lower than those in low grade glioma group(0.87±0.33,1.48±0.22,and 1.91±0.34 respectively)(all P<0.05).The ADC value of thecontralateral mirror region(0.78±0.06)in the high grade glioma group was lower than that in the low grade glioma group(0.78±0.08),the difference was not statistically significant(P>0.05).From the Receiver operating characteristic(ROC)curv,the area under the curve ofCho/NAA,Cho/Cr,NAA/Cr,ADC and rADC were 0.726,0.752,0.793,0.846 and 0.838,which were all higher than 0.5,and were statistically significant.Conclusion:~1H-MRS and DWI all can be used for grading diagnosis of glioma.~1H-MRS combined with DWI in the grading diagnosis of glioma,DWI can supplement the inadequacies of ~1H-MRS.It is benefit for planning clinical comprehensive treatment formulation and assessment prognosis.
