Research On The Demands Of The Community-based Elderly Service Among 280 Aged People In Xuchang,Henan Province

Purpose: Differing from traditional old-age supporting under family and institutions,community old-age service is a pension model that fits the needs of old-age service in current China for allowing the elderly to live in familiar home and community environments while receiving relevant services.It is of great theoretical and practical significance to scientifically and accurately meet community residents' needs for old-age supporting services,to allocate related resources accordingly,to guide and facilitate old-age supporting institutions to provide well-targeted,satisfactory services to the elderly population,to boost the development of the community and home old-age supporting industry,and to effectively address the social problem of aging population in the current stage of society.By conducting a survey of the elderly's need for community old-age services in Xuchang City,Henan Province,this study analyzed the current status and existing problems with community old-age service,as well as the needs of the elderly and their influencing factors,and proposed relevant suggestions and countermeasures to provide theoretical support for further community old-age services improvement.Methods: Residential communities in six counties and districts under the administration of Xuchang City,Henan Province,including Liyuan Community in Xuchang County,Yuhuangge Community,Wangyueqiao Community,Wulangmiao Community and Wenfeng Community in Weidu District,in areas of Xuchang County and Weidu District,were selected by convenient sampling.Through organizing activities for senior citizen,the elderly residents over the age of 60 were intercepted to receive face-to-face questionnaire survey.312 questionnaires have been collected,among which 280 copies were valid.Results:1.In terms of old-age supporting mode selections,more than 86.8% of the elderly expected to spend their lives at home.Age,marriage,education level,personal monthly income,health status,the number of diseases had significant differences influences on the elderly's selection of old-age supporting modes.2.In terms of old-age supporting needs,72.50% of the elderly expect to receive everyday care services,56.10% for health care services,40.40% for sports and fitness services,32.9% for cultural entertainment services and 27.90% for spiritual consolation services.3.In terms of the influencing factors of old-age service,the larger the number of diseases the elderly had,the greater their needs for life care would be.Among elderly residents simultaneously suffering from more than three diseases,88.8% of them needed life care.The elderly who lived alone accounted for 71.4% of those who need health care,higher than the married old-age population(51.6%);with the increasing number they had,the elderly's need for health care also presents a growth trend.The elderly between the age of 60 and 69 had higher cultural entertainment needs(41%)than those aged between 70 to 79(23.0%)and above 80(27.8%);the elderly with higher level of education have higher needs for cultural entertainment;while those who have medical insurance(35.3%)had greater cultural entertainment needs than those who hadn't(20%).The elders with mid-level personal incomes,that was between RMB1001-2000(49.6%)and RMB2001-3000(40.7%),had higher sports and fitness needs than those with lower and higher incomes;whereas,the sports and fitness needs for those who had medical insurance(43.8%)are higher than those who hadn't(22.2%).People aged over 80 had the highest need for spiritual consolation,accounting for 37.0%;with the increase of the number of diseases they had,the elderly's need for spiritual consolation also exhibited a growing trend.4.In terms of the need to purchase long-term care insurance at own expenses,the elderly at education levels of junior middle school(56.0%),senior middle school or technical secondary school(70.0%),undergraduate or junior college and above(44.0%)had higher intentions to purchase long-term care insurance at their own expenses than those whose education levels were elementary school and below(33.3%).The elderly who had hired nannies(83.3%)had lower intentions to purchase long-term care service insurance at their own expenses than those who hadn't(53.0%).The elderly with good health status(67.2%)had higher intentions to purchase long-term care service insurance at their own expenses than those with ordinary(46.3%)and poor(28.6%)health status.The elderly who had medical insurance(57.9%)had higher intentions to purchase long-term care service insurance at their own expenses than those who hadn't(35.6%).5.In terms of the supply of community old-age services,respondents' communities that can provide activity venues accounted for 61.4%,those providing old-age services accounted for 54.3%,those having medical and health services accounted for 79.3%,those having professional volunteers accounted for 59.6%.84.6% of the respondents were willing to spend less than RMB 1000 at a monthly basis to purchase community old-age services,while only 15.4% were willing to spend more than RMB 1000 per month to do so,99.6% of the respondents asserted that the government should enhance its efforts to support home-based old-age care provision services.Conclusion:1.Home care was the main pension model of the elderly to choose.2.The elderly had strong demand for daily life care and medical care,meanwhile,they also had a demand for sports and fitness care,cultural entertainment care,spiritual consolation care and other kinds of care.3.The elderly wished the government enhance its efforts in supporting home-based care in community.4.Government investment should be increased and social participation should be encouraged.Home-based old-age care pattern in community is expected to be focused on public service with private service in support.On the basis of offering daily life care and medical care,services of sports and fitness,cultural entertainment and spiritual consolation should also be provided.Assessment mechanism of care to the aged needs to be established and completed.Long-term care insurance system needs to be developed quickly.Promotion of respecting for the old and caring for the elderly and home-based old-age care needs to be carried out broadly.
