| Background An aging population is a phenomenon of the increasing proportion of older people in the population.Usually in the general population,10% of the elderly aged 60 or 7% old people over the age of 65 years is defined by international as a standards of aging country or area.In 2016,China's statistics yearbook data showed that the country's aged 65 and older reached143.86 million,accounting for 10.5 percent of the total population.In 2017,the 13 th five-year plan for the development and retirement of elderly people in the state of the 13 th five-year plan has proposed higher and newer targets.Such as constructing the home community pension service project;Relying on the comprehensive information platform of public services in urban and rural communities;Integrating the information platform of the home community retirement service;Building call service system and emergency rescue service mechanism;Providing food for the elderly,helping to clean,helping to wash,helping bath,helping medicine,day care and so on;will also implement the "Internet plus" old-age project and build a virtual retirement home.The first virtual nursing home was created,and running in Suzhou,Jiangsu province,in October 2007.Canglang virtual nursing home is one of the typical cases.In December 2009,the first virtual nursing home in northwest China was established in the city of Lanzhou city,Gansu province.Although the national policy support virtual endowment,but there is no research system and evaluating the accepted virtual pension services of elderly people's quality of life and costs.Objective The study through systematic evaluation to clear the elderly people's quality of life of acceptance virtual nursing home care services,and their costs and to provide reference for the development of the virtual pension project and the formulation of national policy.Method In this paper,three methods are mainly used,namely,the review of systematic review,systematic review and the social survey.1)The review of systematic review,evaluation of the existing domestic and international pension model and the evaluation through AMSTAR,and found the theoretical basis for the research.2)System review: using the method of system review and cost analysis of the effects of virtual endowment pattern,mainly from the evaluation of system clear the elderly people's quality of life of acceptance virtual nursing home care services,and analyze the costs;Results Finally,there are 36 system reviews meeting the criteria and evaluated in this review.The systematic reviews were published in 2004 to 2016,with the largest number(6,16.7 percent)in 2013.The average rating of AMSTAR was 5.85,with a range of 1 ~ 10,while the main focus was 6.5(6,16.7 percent),8.5(5,13.9 percent).In the evaluation of the effect and cost analysis of the virtual pension model,13 RCT was included.8(61.5%)report of the quality of life and related indicators,3 of them(23.1%)report the overall ratings of the quality of life,4(30.8%)report the function of body level(functional/physical level)of the grading results.A total of 1,000 questionnaires were issued by the social survey,and the effective recycling rate was 896,and the effective recovery rate was 89.6 percent.The questionnaire had a total of 60 out of 12.12 problems is the total average of 44 points,12 problem of single scored an average of 3.67 points,after adopting factor analysis results show that the virtual virtual old-age pension service attitude,service quality,virtual endowment fee standards,and virtual endowment fee standards for the main influence factors affecting virtual endowment satisfaction.Conclusion(1)The systematic review of the pension model is mainly derived from developed countries.According to the AMSTAR rating,the average quality of the system evaluation is low,and only a small percentage of the system evaluates the criteria recommended by the support guidelines.Unregistered,grey documents,and unstated conflicts of interest are the main reasons for the failure of the theme system to evaluate the AMSTAR.(2)in the system evaluation of the effect and cost analysis of the virtual pension model,13 RCT was included.In terms of quality of life assessment,overall score and patient health status,the difference between virtual and control group was not statistically significant;In the psychological and cognitive state,anxiety and depression scores,virtual endowment group is higher than the control group,while in the body function level,virtual endowment group of score lower than the control group.The use of health care services in outpatient service,emergency treatment,The rate of hospitalization,The rate of hospitalization days,nurse,or a general practitioner visits on the number and the number of respiratory therapists family visit virtual endowment group and control group had no statistical significance of differences;There was no statistically significant difference between the two groups at the rate of mortality,but the virtual group was lower than the control group in the hospital admissions rate.Satisfaction,in 1 ~ 3 months and 6 months,on the overall satisfaction score of virtual endowment group is higher than the control group,and in September and December's overall satisfaction evaluation,health care institutions users or assisted living institutions on the internal and external system satisfaction of old people,there was no statistically significant difference of the two groups.Cost,in this medical expenses,examination and medical clinic and hospitalization expenses,nurses home visiting on the cost,the respiratory therapists family visit cost and total cost were no statistically significant difference of the two groups. |
