Study In Social Work Case Management

Social development has bringing us more wealth,meanwhile,it has also impose a lot of pressure on people’s lives as well as environmental pollution.A variety of reasons led to the growing population of cancer patients in China,cancer not only bring to patients with physical,psychological and economic pressure,but also change the entire family members of the original role of cognition,leading to family problems.Therefore,the services for cancer patients should be comprehensive.However,the current mode of the hospital is difficult to meet the needs of such patients.As a self-help profession,social work can be used to provide diversified services for patients from the individual level,family level,social level,links a variety of social resources,make up for the lack of hospital services.This study enriches the application of case management model in social work practice,intervening patients from multiple dimensions,helping patients to reduce the adverse problems caused by illness,to re-establish the correct self-awareness,strengthen the social support system.This study can help to expand the influence of social work,provide practical reflection and suggestions on the social work intervention of cancer patients,and provide some basis for the practice and development of medical social work.This study is based on case management theory,advantage perspective theory,the theory of social support system and a lot of case management literature.A breast cancer patient in the Department of Oncology of X city,Shaanxi Province was the main research object.Firstly,The article analyzed all breast cancer patients in hospital within three months with POMS-SF and the self-perceived burden scale.Secondly,the researcher intervened to help the client to ease the mood,rebuild confidence,change its inherent irrational awareness;by helping the client find their own advantages,to rebuild their social support system to improve the family relationship and provide medical,financial resources.Finally,based on the practice of case management intervention,the article reflect the shortcomings of the intervention process,as well as the theoretical and methodological reflection of case management,the advantage perspective theories in the process of breast cancer intervention The article puts forward some countermeasures and suggestions to improve the service system of breast cancer patients,including perfecting the case management service system,bringing the families of cancer patients into the service,promoting the integration of social resources and constructing the social support network of cancer patients.
