The Effect Of Restrictive Infusion On Recent Clinical Outcomes After Gastric Cancer Surgery

Objective: To observe the postoperative gastric cancer patients restrictive transfusion in the recent clinical outcomes.Methods : Selection of Qingdao municipal hospital affiliated to Qingdao university general surgery between August 2014 and October 2014 deadline to open most of gastric cancer radical distal gastric resection + Roux en-Y anastomosis of patient 80 cases.The male 56 cases,female 26 cases,aged 35 to 87 years old.Method 80 cases of gastric cancer patients according to the random form is divided into conventional infusion control group 40 cases and the limitation of th e restrictive infusion group of 40 cases,Dynamic monitoring to compare two groups of patients with postoperative transfusion volume for 4 days,postoperative fever time,complications post-operation anus exhaust time defecate,the number of cases(infection of incision,incision dehiscence,anastomotic fistula,arrhythmia,pulmonary infection,respiratory failure,etc.).Postoperative hospitalization days and the white blood cells,c-reactive protein(CRP),hemoglobin,aspertate aminotransferase(AST),cereal third transaminase(ALT),glucose,urea nitrogen(BUN),serum creatinine(Cr),the BNP,etc.Results: Two groups of patients with preoperative comparing white no statistical difference(P > 0.05),postoperative on the first day of the control group f or blood cell is(18.14 ± 3.87)(10 ? 9 / L),limit the number of group of white blood cells is(15.02 ± 2.31)(10 ? 9 / L),comparing two groups of data statistically significant(P < 0.05).After the second day of the control group for blood cell is(14.58 ± 2.65)(10 ? 9 / L),limit the number of group of white blood cells is(12.41 ± 1.75)(10 ? 9 / L),comparing two groups of data statistically significant(P < 0.05).Postoperative day 7 in the control group for blood cell is(8.21 ± 1.32)(10 ? 9 / L),limit the number of group of white blood cells is(7.67 ± 1.26)(10 ? 9 / L),comparing two groups of data with no statistical difference(P > 0.05).Two groups of patients with postoperative leukocyte compared preoperative elevated obviously.C-reactive protein(CRP)in the third day after peak respectively(164.20 ± 30.22 mg/ml),(141.65±22.06 mg/ml)and has a statistically significant difference(P < 0.05).Control and limit the BNP compared to 1,3,7 days postoperatively(4.16 ± 1.25),(4.27±1.22),(4.19±1.18)pg/ml and(4.27 ± 1.31),(4.23± 1.30),(4.26± 1.27)pg/ml.Two groups of postoperative BNP compared with preoperative rise,but still within the normal range.Control and limit postoperative 1,3,7 day BNP difference was statistically significant(P < 0.05),routine postoperative BNP more restricted group increased significantly.And compared the two groups have to hemoglobin,aspertate aminotransferase(AST),cereal third transaminase(ALT),blood urea nitrogen(BUN),serum creatinine(Cr)were no statisti cal difference of preoperative and postoperative limited group of patients with postoperative fever time,anal exhaust time,hospitalization days defecate significantly less than the control group(P < 0.05),less postoperative complications happened.Conclusion: The gastric cancer patients postoperative patients with restrictive transfusion can accelerate recovery,reduce the incidence of postoperative complications.
