| ObjectiveTo explore the characters of clinical dose and application of Shi Zao Tang(SZT),analyze and discuss about the regulation and influence of different factors on the dose via searching and studying the former literature of SZT.MethodsLiterature screening and data extraction were carried out by two researchers using hands and computers to retrieve in the fifth edition of the Encyclopaedia of Traditional Chinese Medicine, the book retrieval platform of the Beijing University of Chinese Medicine library, the China National Knowledge Infrastructure, the Wan Fang data knowledge service platform and the CQVIP Chinese Journal Database.Medical cases, medical notes and clinical reports on SZT written by practitioners from the Han Dynasty to present were included in this study. The valid data was filtered out and input into Microsoft Excel 2016 to build the database.SPSS 20.0 software was used to make statistics. The measurement data of normal distribution were based on t-test or t’-test according to the test results of homogeneity of variance. The non normal distribution of the measurement data was tested by nonparametric test, and the counting data of the two classified variables was checked by Chi-square test.Results1.Study on the dosage of Shi Zao TangThe description of total and single taken dose of gan sui,da ji and yuan hua in one dose of SZT is as follows:In Han,Tang and Song Dynasties, the dose obviously increases and then decreases; In the Song Dynasty and later, the most commonly used quantity tends to be stable, and the range of commonly used dose tends to be concentrated, but the general amount is still higher than that in the Han Dynasty. The total and single taken dose of gan sui,da ji and yuan hua in one dose of SZT are mostly higher in the decoction group than those in the powder group. The range of external used dose of gan sui,da ji and yuan hua is extremely wide. This situation is related to the specific way of external use. Therefore, the specific external used dose of the SZT should depend on its usage.2.The dose of single herb in Shi Zao TangIn the horizontal analysis, the tendency of the dose of gdn sui,da ji and yuan hua is the same, firstly going up and declining, and then becoming stable after the Song Dynasty, but the level of the dosage is higher than that of the Han Dynasty;Vertically, the dose of gan sui,da ji and yuan hua is nearly the same, and the range of the dosage of da ja is slightly wider than gan sui and yuan hua;The average dose of da zao is basically consistent with that of Han Dynasty, and the range of dose is quite wide in the Jin and Yuan Dynasties and modern times. The ratio of dose of substances:gan sui:da ji:yuan hua=1:1:1 is the common case;the ratio of da zao to gan sui,da ja and yuan hud in turn decreases from Han Dynasty and the highest frequency ratio is only 1/6 of it in the Han Dynasty.3.The characteristic of clinical application of SZT In the past dynasties,SZT was used as power and taken on an empty stomach, following the usage of the Han Dynasty;Attention should be paid that once the effect occurs, the decoction should be stopped immediately, and the effect might occur within 4 doses. The function of the postposed herbs in SZT are mainly tonifying and alleviating water, which is similar with original formula, thus indicating the using of mutual promotion of herbs.4.The regulation factors of the modern dose of Shi Zao Tang In modern clinical application of SZT,the proportion of males is significantly higher than that of females, and the proportion of adults is significantly higher than that of elders and minors. The disease requiring SZT is mostly relevant with fluid-retention,for Shi Zao Tang can alleviate the water in cavity gap,in or among the tissues, not just the visible extra fluid. The clinical treatment of SZT is mostly related to water retention,it can not only eliminate the visible excess liquid, but also the excess liquid in the lacuna and between or in the tissues.ConclusionThe clinical dosage of SZT has been basically stabilized through the practice of successive generations of doctors. When used as a dispersant, the commonly used amount of the total dosage of gan sui.da ji and yuan hud is 2-4g,the most commonly used amount of da zao is 30g,and the ratio is 10.When used as a decoction, the dosage needs to be increased. |
