ObjectiveIn this study,we summarized the characteristics of Syndromes element of MPN,to get the TCM syndromes and pathogenesis of MPN.To explore the advantages of traditional Chinese medicine in the management of MPN.MethodsStudy 1:Using prospective research methods, the objects of this study were 123 patients with BCR-ABL negative MPN in the Department of Blood Oncology of Dongzhimen Hospital from January 2017 to March 2018 and Peking Union Medical College from September 2017 to October 2017.123 patients filled out the MPN syndromes collecting form and MPN TCM clinical observation table, analyzed the basic information of the patients and the characteristics of the relevant syndromes element, and obtained the relevant syndromes and pathogenesis characteristics of MPN.Study 2:The objects of study were 58 MPN patients treated with traditional Chinese medicine in Department of Blood Oncology,Dongzhimen Hospital. After filled out the TCM clinical observation form of MPN,the prescription depanded on the differentiation of symptoms. The duration of prescription was from 1 month to 3 months(The patients with MF were treated for 1 month, patients with PV and ET were treated for 3 months.4 cases were lost in midway due to individual reasons. After full course of treatment,54 patients' symptom score and blood index were counted.Case control analysis was used to analyze the changes of symptom scores and laboratory indexes of MPN before and after treatment with traditional Chinese medicine(TCM)in order to verify the effectiveness of traditional Chinese medicine(TCM)in alleviating the symptoms of the patients.Results1 The symptoms of 123 patients with MPN were analyzed by descriptive statistics.127 symptoms were found in all patients, among which 18 symptoms appeared more frequently than 10 times. The percentage of symptoms element that more than 5%,including heat,qi stagnation, yang deficiency, liver, blood deficiency, dampness,qi deficiency, yin deficiency, among which the largest symptom factor of disease location is liver and the largest symptom factor of disease nature is yin deficiency.Cluster analysis showed that the TCM syndromes were yin deficiency syndrome, dampness syndrome,qi and blood deficiency syndrome and liver depression and fire syndrome. The higher syndromes element of PV patients were yin deficiency, liver, blood deficiency, phlegm and heat. The higher syndromes element of MF patients were spleen,qi stagnation, yang deficiency, dampness and qi deficiency.2 A total of 54 patients treated with traditional Chinese medicine were counted.Including 42 ET patients,6 PV patients and 6 MF patients. The mean scores before and after treatment of ET were 15.52±6.21 and 9.67±3.99;mean scores before and after treatment of PV were 18.67± 10.25 and 10.33±3.67,mean scores before and after treatment of MF were 20.00±6.69 and 10.33±2.94.The average total score before and after operation was 16.37±6.83 and 9.82±3.81.There was significant difference in post-score between before and after treatment(P<0.05).There was significant difference in PLT of ET patients between before and after treatment(P<0.05).There was no significant difference in HGB,HCT of PV patients between before and after treatment(P>0.05).There was significant difference in HGB of MF patients between before and after treatment(P<0.05).There was no significant difference in PLT,WBC of MF patients between before and after treatment(P>0.05).Conclusion1 In this study, the largest symptom factor of disease location is liver and the largest symptom factor of disease nature is yin deficiency. The TCM syndromes were yin deficiency syndrome, dampness syndrome,qi and blood deficiency syndrome and liver depression and fire syndrome.2 The intervention of Chinese medicine on MPN can relieve the symptom load of the patients, which shows that it has some advantages in symptom management.
