| Objective:By collecting the relevant information of patients with stroke in our hospital,to analyze the nutritional status of inpatients with stroke and the related risk factors of nutrition risk.Methods:The stroke patients who were hospitalized in the Department of Neurology of China-Japan Union of Jilin University from January 2016 to December 2017 were selected as the study subjects.All patients met the inclusion criteria and discharge criteria,and were all completed by professionals within 24 hours after admission.Self-designed case report form.After the case report form was collected,EpiData software was used for data entry.SPSS23.0 software was used for statistical analysis of the data.Quantitative data were expressed as mean±standard deviation,and count data adoption rate,composition ratio,etc.were expressed.Univariate analysis was performed using t test and X2 test.Logistic regression model was used for multivariate analysis.When P<0.05,the difference was statistically significant.Results1.General information: 70 males,accounting for 68.0%,with an average age of 61.67±11.23 years and 33 females,accounting for 32.0%,with an average age of 65.70± 11.45 years.2.Investigation on admission: 92 cases with cerebral infarction,11 cases with cerebral hemorrhage,72 cases with one chronic disease,accounting for 69.9%,14 cases with 2 chronic diseases,accounting for 13.6%,and 17 cases without complications,accounting for 16.5%.For the first time,74 cases accounted for 71.8%.There were 69 patients with a history of hypertension(65.0%),8 patients with a history of heart disease(7.8%),and 23 patients with a history of diabetes(22.3%).3.Neurological function scores,SAS scores,and SDS scores: Among the neurological function scores,75 were light strokes and 28 were moderate strokes.Among the anxiety scales,97 were anxiety-free and 6 were mild anxiety.No scores were found in depression scales.In 61 cases,33 cases were mild depression,8 cases were moderate depression,and 1 case was severe depression.4.NRS-2002 Nutrition Risk Screening: 103 patients with stroke who met the inclusion criteria completed NRS-2002 for nutritional risk screening.The incidence of nutritional risk was 51.5%,of which males accounted for 33.0% and females accounted for 18.4%.70 The probability of occurrence of nutritional risk in males was 48.6%,and the probability of occurrence of nutritional risk in 33 females was 58.0%.The probability of females suffering from nutritional risk was slightly higher than that of males.5.Malnutrition statistics: There were 10 patients with malnutrition,accounting for 9.7%,and 93 patients without malnutrition,accounting for 90.3%.6.Nutritional support: 11 nutritional supporters were given,nutrition support rate was 10.7%,6 patients were parenteral nutrition,4 were enteral nutrition,and 1 was enteral nutrition combined with parenteral nutrition.There were only 5 cases of nutritional risk given to nutritional support and 6 cases of no nutritional risk given to nutritional support.7.Statistics of clinical outcomes: There were 8 patients with infectious complications during hospitalization,accounting for 7.8% of the total number of patients included in the study,and 2 patients with other complications(abdominal pain,lower extremity edema,etc.),accounting for 2.0%.Thirty-one patients(30.1%)were admitted to the intensive care unit(ICU)during hospitalization.8.According to the results of univariate analysis of the risk factors of nutritional risk in patients with stroke,the two groups of patients had no significant differences in their height,average income,gender,marital status,occupation,ethnicity,living style,educational level,type of medical insurance,Breakfast,eating habits,the type of staple food,whether they often eat fresh vegetables,whether they often eat fresh fruits,whether they often eat meat,whether they often consume fish and shrimp and other aquatic products,whether they often consume eggs,whether they often consume soy products,smoking history,Drinking history,whether exercise,whether to ensure sleep 7-8 hours per night,type of stroke,secondary diagnosis,stroke,whether the first incidence,history of the past,triceps skinfold thickness,waist circumference,hip circumference,waist hip There was no significant difference in the indexes of the upper arm circumference,upper arm circumference,body temperature,heart rate,blood pressure,edema,anxiety,hemoglobin,albumin,total protein,prealbumin,globulin(P>0.05).Age,weight,number of staple food,whether or not regular consumption of eggs,past history of diabetes mellitus,upper arm muscle circumference,neurological deficit and whether depression and other indicators in the two groups were statistically significant(P <0.05).9.Multivariate logistic regression analysis of risk factors for nutritional risk in hospitalized patients with stroke:The independent risk factors for nutrition risk of hospitalized patients with stroke were age,neurological deficit,depression(P<0.05)(OR = 1.078,4.092,3.506).Conclusion:1.The incidence of nutritional risk in 103 patients with stroke was 51.5%,and the incidence of malnutrition was 9.7%.2.Inpatients with stroke have a lower nutritional support rate(10.7%)and nutritional support methods are not uniform.3.Independent risk factors of nutritional risk in patients with stroke are age,degree of neurological deficit,whether or not depression,the degree of neurological deficit is the main factor in the three,followed by whether or not depression. |
