| Purpose:To analyze the clinical efficacy of single and dual locking compression of steel plate in the treatment of adult tibial fractures.Methods:From September 2015 to May 2017,88 cases of tibia-fibula fracture was admitted in the inpatient department of Orthopaedic in our hospital,the single plate treatment group(n=42)and the medial and lateral plate group(n=46).The same surgical approach was used to compare the time required for surgery,the amount of blood loss during surgery,the length of hospitalization and fracture healing,and postoperative complications.All patients were followed up for 1 month,3 months,6 months,12 months and 20 months,with an average of 18 months.Both groups used Johner-Wruhs tibial fracture efficacy evaluation criteria,HSS knee function score criteria,The AOFAS ankle evaluation criteria were scored.Results:1.There was no significant difference between the two groups in age,gender,causes of injury,site of disease,injury to operation time,and AO/OTA classification of tibial fractures(all P>0.05).2.The operation time of the single plate group and the double plate group were 99.09±3.18min and 99.88±2.92min respectively,and the intraoperative blood loss was 48.61±4.45ml and 49.36±5.35ml.There was no statistical difference between the two groups(all P>0.05).The duration of hospitalization of patients in the single plate group and the double plate group were 7.95±0.70d and 3.90±0.70d,and the fracture healing time was 99.55±4.73d and 70.62±4.39d.There was a statistically significant difference between the two groups(P<0.05).The incidence of postoperative complications was 28.57%and 8.68%respectively.There was a statistically significant difference between the two groups(P<0.05).3.Single and double plate group Johner-Wruhs tibial fracture scores:excellent rate were 76.19%and 93.48%respectively,there was statistically significant difference between the two groups(P<0.05).HSS knee function score of single and double plate groups were 80.95%and 91.30%,respectively.There was a statistically significant difference between the two groups(P<0.05).The scores of AOFAS ankle joint function in the single and double plate groups were 72.09%and 88.13%respectively.There was a statistically significant difference between the two groups(P<0.05).Conclusion:In the treatment of adult patients with tibial fractures,the use of double-locking compression plate can achieve primary healing of fractures,firm fixation,early functional exercise,and reduce the incidence of postoperative complications. |
