OBJECTIVE:Using meta-analysis method to evaluate the efficacy of Alprostadil on diabetic retinopathy.METHODS:The use of computers to search a variety of electronic databases,including China Biomedical Literature Database(CBM),Chinese Science and Technology Journal Full-text Database(VIP),Chinese Journal Full-text Database(CNKI),Wanfang Data,PubMed Database,OVID Database,the Cochrane Library database,the retrieval time is set from building the database until December 2017.Fifty-six articles related to diabetic retinopathy treatment with alprostadil were obtained through computer-based retrieval.Using RevMan 5.3 software to screen related studies,eight clinical randomized controlled trials(RCTs)that met the inclusion exclusion criteria were used in this study,totaling 635 patients.316 cases in the experimental group,319 cases in the control group.Jadad’s score criteria were used to evaluate quality of the articles.RESULT:Because there are few studies on the treatment of diabetic retinopathy with alprostadil,the number of included literature is relatively small,and the results show that the overall quality is not high,but there is no obvious high risk of bias.A comparison was made between the efficacy of the 5 study groups and the control group.Meta-analysis showed that the included studies had no statistically significant heterogeneity,statistically significant,with combined OR(95%Cl)of 3.27[2.10,5.11];a comparative analysis of two glycosylated hemoglobin(HbA1C)after treatment.The meta-analysis showed that there was no statistically significant heterogeneity in the 2 studies included,and the combined MD(95%Cl)was 0.05[-0.46,0.57].Two studies mentioned the number of retinal hemorrhages in both the experimental group and the control group,and compared the number of retinal hemorrhages before and after treatment in the two groups,showing the results of comparison of retinal hemorrhage spots between the two groups.Meta-analysis showed that the two studies included had statistical heterogeneity,but no clinical factors leading to heterogeneity were found.Statistically significant,the treatment regimen in the experimental group was superior to the control group.Two studies mentioned the number of fundus hemangiomas in the patients of the experimental group and the control group and compared the number of fundus hemangiomas before and after treatment in the two groups,showing the results of comparison of the number of fundus hemangioma between the two groups.The meta-analysis showed that the two studies included had no statistical heterogeneity and statistical significance.The treatment regimen in the experimental group was superior to the control groupCONCLUSION:The effect of alprostadil on improving or stabilizing the condition of patients with diabetic retinopathy is visible.The results show that alprostadil has a certain therapeutic effect on diabetic retinopathy and provides a direction and basis for further clinical research.Since there are few studies on the treatment of diabetic retinopathy with alprostadil,the number of included literature is relatively small,and the results show that the overall quality is not high and there are certain limitations,so more RCTs and more high-quality studies are needed.
