Objective To investigate the relationship between quantitative score of Sonographic image signs and ultrasound breast imaging reporting and data system(US-BI-RADS)classification in breast lesions,and to discuss the value of multimode ultrasound correcting US-BI-RADS classification.Methods(1)To score 370 breast lesions based on 11 sonographic image signs,and the lesions were classified according to the scores.The pathological results after surgery were selected as golden standard,the malignant predicted value was compared with theorized risk degree of malignancy by US-BI-RADS to obtain the relationship between score and US-BI-RADS.A ROC curve was drawn to get the ideal point to differentiating breast benign and malignant breast lesions.(2)370 breast lesions were classification diagnosed for routine ultrasound US-BI-RADS and ultrasonic image signs quantitative scoring US-BI-RADS.A ROC curve was used to compare the diagnostic value of routine ultrasound US-BI-RADS and ultrasonic image signs quantitative scoring US-BI-RADS.(3)Apply multimode ultrasound for correcting 116 breast lesions US-BI-RADS classification,and draw a ROC curve to compare the differential diagnosis of breast lesions between ultrasonic image signs quantitative scoring classification and multimode ultrasound correcting classification.Results(1)The 3,4A,4B,4C and 5 by US-BI-RADS were corresponded to score 0-3,4-6,7-8,9-11,12-15,respectively.Score 8 is idealpoint to differentiate breast benign and malignant breast lesions,the sensitivity were 90.4%,specificity were 90.6%,accuracy were 90.5%,positive predictive value were 89.9%,negative predictive value were 91.1%.(2)These sonographic images signs with higher positive predictive value in diagnosis of breast cancer for three former were as follows:burr or Crab-like edge,very irregular shape,microcalcification,the positive predictive value was 95.5%,91.8%,91.7%,respectively.(3)The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of routine ultrasound US-BI-RADS for 370 breast lesions in the diagnosis of breast cancer were 93.3%,70.8%,81.6%,74.8%,91.9%,respectively.Those of ultrasonic image signs quantitative scoring US-BI-RADS were 92.7%.85?9%?89.2%?85.9%?92.7%,respectively.The area under ROC curve for ultrasonic image signs quantitative scoring US-BI-RADS was 0.963,which was higher than the area of routine ultrasound US-BI-RADS which was 0.899,there was statistically significant between two methods(Z=2.907,P<0.05).(4)The sensitivity,specificity,accuracy,positive predictive value and negative predictive value for ultrasonic image signs quantitative scoring US-BI-RADS classification of 116 breast lesions were 91.9%?87.0%?89.7%?89.1%?90.4%,respectively.Those of multimode ultrasound correcting US-BI-RADS classification were 96.8%?88.9%?93.1%?90.9%?96.0%,respectively.The area under ROC curve for multimode ultrasound correcting US-BI-RADS classification was 0.957,which was higher than the area of ultrasonic image signs quantitative scoring US-BI-RADS classification which was 0.953,there was statistically significant between two methods(Z?2.035,P<0.05).Conclusion(1)Ultrasonic image signs quantitative scoring can make the US-BI-RADS classification to be objectivit and standardization.(2)There is the correspondence between ultrasonic image signs quantitative scoring and US-BI-RADS,Score 8 is the idealpoint to differentiating breast benign and malignant breast lesions.(3)These sonographic image signs of burr or Crab-like edge,very irregular shape,microcalcification was sensitive indicator in diagnosis of breast cancer.(4)Ultrasonic image signs quantitative scoring improve diagnostic performance of US-BI-RADS classification.(5)Multimode ultrasound correcting US-BI-RADS classification can further improve the diagnostic performance.
