Discussion On Diagnosis Value Of Echocardiography On Cardiac Amyloidosis

Objectives:1.To analyze and conclude clinical features and noninvasive diagnosis method of cardiac amyloidosis(CA).2.To discuss the diagnosis value of echocardiography on cardiac amyloidosis.Methods:28 patients suspected with cardiac amyloidosis served as CA group(8 cases which confirmed by heart outside biopsy served as CA1 group;20 cases which highly suspected of CA with clinical and echocardiography-served as CA2 group),a retrospective analysis was made on clinical features and noninvasive diagnosis methods from CA group,especially characteristics of echocardiography;20 healthy individuals were taken as the normal control group.Statistical analysis was carried out on echocardiography indexes of each group.Results:1.Clinical manifestations,Laboratory results and electrocardiogram performances of cardiac amyloidosis have no specificity.2.Echocardiography of patients from CA group:(1)typical manifestations:concentric hypertrophy and cardiac muscle shining syndrome in the interventricular septum and left ventricular wall;(2)echocardiography indexes analysis:compared with the normal control group,IVS,LVPW,LATD of CA1 group increased,LVDd decreased;compared with the normal control group,IVS,LVPW,LAD,LATD of CA2 group increased,LVEF decreased;compared with CA1 group,LAD of CA2 group increased,LVEF decreased;compared with the normal control group,Ea?Aa?Ea/Aa of 6 patients suspected with CA decreased,Tei index of left ventricle increased.In the above comparisons,P<0.05.Conclusions:Cardiac amyloidosis of the clinical manifestations are lack of specificity,the pathological biopsy is difficult to routine.Echocardiography can directly show the heart form and echo typical changes of patients with cardiac amyloidosis and assess their diastolic and systolic functions,so it is of important clinical value for diagnosis and prognosis assessment.
