Objective:Studies have shown that high blood pressure, diabetes, obesity, infertility are high risk factors for endometrial carcinoma. While polycystic ovary syndrome may increase the risk of endometrial cancer, there are few prospective studies of their relevance. In this study,we retrospectively studied the clinical endocrine and metabolic characteristics of patients with endometrial carcinoma diagnosed by surgical operation in our hospital in recent 11 years, and explored the correlation with polycystic ovary syndrome(PCOS).To intervene or reduce the incidence of endometrial cancer due to metabolic causes such as polycystic ovary syndrome. Methods: 1.From January 2006 to November 2016,the patients with endometrioid adenocarcinoma in our hospital were collected. All the cases were confirmed by histopathological and immunohistochemical examination. In addition, the patients with hysteromyoma in the same period(selected by systematic sampling)were selected as the control group, and the demographic data, including age, were collected.Weight and height calculate body mass index;menstrual history, hypertension, diabetes history;fasting blood glucose, blood lipid;and menstrual history,Kaohsiung manifestations such as hirsutism, and ovarian polycystic features(including ultrasound hint polycystic, intraoperative ovarian polycystic,Postoperative ovarian pathological polycystic, the results to collect relevant information.2.Inclusion criteria: patients with endometrial adenocarcinoma and hysteromyoma were diagnosed, curettage and recurrence after hysteroscopic surgery in our hospital.3.The lack of data in the telephone follow-up records included the patient’s height and menstrual history, past history.4.All the data were processed by SPSS19.0 software, the measurement data were expressed as mean ±standard deviation(x ±s),the two groups were compared by t test, and the count data were statistically significant by χ 2 test(p< 0.05).Results: 1.733 patients with endometrial carcinoma were admitted from January 2006 to November 2016 in our hospital. All the cases were confirmed by pathological examination. The onset age was 28-87 years old,the average age was 58.7 years.Since the average age of menopause in women in China is 49.5 years, the incidence of endometrial cancer after menopause has increased significantly. Therefore, the age is divided into two groups: less than 50 years old and 50-60 years old.A total of 130 patients(<116 in Type 1)were included in the 50-year-old group, and 302(50-60-yearold)patients in the 50-60-year-old group(266 of them were type 1).At the same time, patients with uterine leiomyoma at the same period were selected as the control group.1860 cases were less than 50 years old(150 cases were selected by systematic sampling),578 were 50-60 years old,and 294 were randomly selected.2.In the group of patients with endometrial cancer less than 50 years old,111 cases of type 1 endometrial cancer were compared with 138 cases in the control group, the mean age was 42.4±5.8 years and 41.9±5.7 years respectively, with no statistical difference;(1)Type I Endometrial cancer body mass index(BMI)≥25kg/m2 was more than control group, with statistical difference(p=0.0034);(2)The proportion of diabetes history was higher than that of control group, with statistical difference(p=0.004).The fasting blood glucose(5.8±2.5mmol/L)in endometrial cancer was higher than that in control group(4.9±0.8mmol/L),with statistical difference(p=0.000);(3)The triglyceride in blood lipid had statistically significant Differences in learning(p=0.006);(4)Patients with thinning menstrual weeks in the endometrial cancer group were higher than those in the control group(p=0.000);(5)Not in the number of pregnancies in the 50-year-old group The ratio of pregnancy and infertility was also higher than that of the control group(p=0.00,0.004).There was a statistically significant difference. The proportion of unparticipated childbirth was higher than that of the control group, but there was no statistical difference;(6)There was no statistical difference in the history of hypertension;3.There was no significant difference in BMI between 50 and 60 years old patients with endometrial carcinoma compared with the control group(P < 0.081)fasting blood glucose was significantly higher than that in the control group(P < 0.05),but the proportion of hypertension, diabetes mellitus and leiomyoma group was higher than that of the control group, although there was no statistical difference between the patients with hypertension, diabetes mellitus and leiomyoma. There was no significant difference in triglyceride in blood lipid compared with control group, total cholesterol, high density lipoprotein, low density lipoprotein, and no significant difference in menstrual cycle compared with control group.4.111 patients with type I endometrial carcinoma younger than 50 years old,There were 13 cases of polycystic ovary diagnosed by ultrasound and 1 case of polycystic ovary perforation. One case was diagnosed by polycystic ovary drilling in hospital.4 cases of ovarian polycystic ovary were confirmed by pathology after operation.6 cases were diagnosed by pathological section, accounting for 13.3%,myoma patients. In one case, the ovary was found to be polycystic during the operation. There were no cases of polycystic ovary in the age group of 0.0000 and 50-60 years old. Conclusions: 1.The proportion of obese type I endometrial carcinoma patients with diabetes history was higher than that of hysteromyoma control group, and the proportion of hyperlipidemia(triglyceride and total cholesterol)group was higher than that of control group.2.In the group less than 50 years old,the proportion of endometrium cancer patients with dilute menstruation, polycystic ovary and infertility was higher than that of the control group, and the index of polycystic ovary was higher than that of the control group.3.In the 50-60-year-old group, the history of diabetes, fasting blood glucose, triglyceride, and infertility in patients with endometrial cancer were significantly different from those in the control group;BMI,hypertension, and number of pregnancies were not statistically different from those in the control group.4.In the 50-60-year-old group, the proportion of infertility patients with endometrial cancer was higher than that of the control group. There was no statistical difference in menstrual thinning compared with the control group.
