The Study On Microcosmic Syndrome Differentiation In The Phlegm-blood Stasis Syndrome Of Idiopathic Membranous Nephropathy Patients

Objective: To observe the renal pathology and laboratory indexes in the phlegm-blood stasis syndrome of idiopathic membranous nephropathy(IMN)patients,in-depth discussion of clinical pathological and laboratory indexes changes in the phlegm-blood stasis syndrome of IMN patients.To provide a quantitative reference index for microcosmic syndrome differentiation of idiopathic membranous nephropathy,and to provide the basis for TCM syndrome differentiation of IMN.Methods: 60 cases of IMN patients who met the inclusion criteria of the course were screened.The renal pathological results of the patients were evaluated by reference to the Katafuchi pathological scoring system.TCM syndromes of phlegm-blood stasis syndrome were quantified by reference to The Guiding Principles for The Clinical Research of New Drugs in Chinese Medicine(trial)by Zheng Xiaoyu in 2002.At the same time,the results of laboratory tests in 60 patients with the same period were collected.The correlation analysis was carried out in 60 cases of IMN patients with the syndrome differentiation of phlegm-blood stasis and the gender,the age,the course of the disease,the pathology and the laboratory index.All data was processed by using statistical software SPSS 19.0,while P(27)0.05 is considered statistically significant,and P(29)0.05 for the difference that is not significant.Results:(1)This study participants included 60 patients,42 cases of men,women 18 cases,the ratio of men and women is 2.33:1.60 cases of the patients in the mean age of 52.98 years,the standard deviation was 13.41 years.Male patients in the mean age of 54.57 years,the standard deviation was 12.50 years,women in the mean age of 49.28 years,the standard deviation was 15.03 years,showed that the age of male patients is similar to that of female patients patients.(2)There was a statistically significant difference between male and female patients with the scores of phlegm-blood stasis syndrome(P(27)0.05).(3)There was a statistically significant difference between the scores of phlegmblood stasis syndrome in patients with different age groups(P(27)0.05),including50-79 aged patients with the scores of phlegm-blood stasis by the highest.(4)There was a statistically significant difference between the scores of phlegm-blood stasis syndrome in patients with different course of the disease(P(27)0.05),more than 5 years duration period of phlegm-blood stasis in patients with each other by the highest.(5)There was a statistically significant difference between the scores of phlegm-blood stasis syndrome in patients with different renal pathological stages(P(27)0.05).(6)The scores of phlegm-blood stasis syndrome in patients respectively with the glomerular hardening integral,the renal tubuleinterstitial injury integral,the renal small vessel lesion integral,the renal pathological chronicity index to compare,a positive linear correlation existed between them(P(27)0.05).(7)To compare the relationship between laboratory indexes of patients with different renal pathological stages,shows that there was a statistically significant difference between the renal pathological stages and serum creatinine,24 hour urine protein quantitative,blood triglycerides,total cholesterol(P(27)0.05).There was no statistically significant difference between the renal pathological stages and albumin,uric acid,serum Ig G,serum Ig A,serum Ig M,serum C3,Serum C4(P>0.05).Conclusion: Among the IMN patients with the phlegm-blood stasis syndrome,there were more men than women.The older,the longer courses was positively related to the degree of phlegm-blood stasis.It was in line with the theory that TCM "A long illness kidney empty" and "A long illness blood stasis".The TCM pathogenesis of "phlegm-blood stasis" can be used as an objective basis for pathological changes of IMN,Phlegm and stasis obstructing collaterals are important pathological changes of IMN.The increase of serum creatinine,24 hour urine protein quantitative,blood triglycerides,total cholesterol may be closely related to the progression of idiopathic membranous nephropathy to phlegm and blood stasis,or it can be used as a reference index for syndrome differentiation of phlegm and blood stasis syndrome in this disease.
