Study On The Characteristics Of Hospitalized Cases Of Accidental Injury To The Elderly In Rural Cooperative Medical Service

Background:With the ageing of population development and longer lifespans,the healthiness and life quality of the elderly has become a social concern,their public health problems occupies an important position.WHO has placed injury、communicable diseases and non-communicable diseases as the three main public health problems society faces today.Injury has risen to the fifthObjective:According to the characteristics of the elderly in the new rural cooperative medical system,the distribution of the characteristics of human and accidental injuries was investigated.Objective to analyze the hospitalization situation in order to obtain the characteristics of the key population and key injury,so as to carry out targeted intervention,provide clues and etiological hypotheses for accidental injury,and provide the basis for the analysis of epidemiological studies.Method:From 2013-2014 years all NCMS elderly people have accidental injuries of the original case data,the original data were input into the database after encoding;statistical analysis using SPSS 20 software,that whether there are significant differences between the groups results;through logical analysis,to reveal the distribution of accidents in the new rural cooperative medical insurance in the elderly,and analysis of the influencing factors,and through the comparison of the total medical costs of hospitalization days and the various influence factors and cost,to find the rising medical costs.Results:1.486 cases of new elderly accident cases,sex ratio of 0.49:1,more women than men,65~69 years of age the most cases,male patients with 65~69,aged 70~74 years old in the two age of majority,female patients with 60~64,aged 65~69 years old in the two age.The most frequent injuries occurred during the day(6:00-18:00),accounting for 71.25%.Most of the elderly patients were 70-79 years old(18:00-24:00)who had accidental injuries.The number of accidental injuries in 6-10 months accounted for 48.35% of the total cases,the most in July,and more injuries in summer and autumn than in winter and spring.2.The causes of injuries in all cases were fall(55.35%),traffic injuries(31.07%),sharp injuries(4.73%),crush injuries(3.09%),others(falls,burns,animal injuries,etc.)(3.29%)and hard injuries(2.47%).Falls and traffic injuries constitute the largest proportion,accounting for 86.42% of the total number of cases,which is the main cause of the elderly inpatients with accidental injury in the new rural cooperative medical system.There was statistically significant difference between different gender and age groups(P(27)0.05).3.Among all cases,walking was 1.44%(40.12%),riding electric bicycle / bicycle(31.69%),daily living(9.67%),housework(8.64%),work / farming(8.44%),and others(2).There were significant differences between different genders and age groups(P(27)0.05).4.All the injured sites were ranked as home / residence(46.30%),road / bridge(39.09%),farmland / work place(8.23%),public place / community / other(6.38%).The number of women who had accidental injuries at home / residence was 3.17 times higher than that of men.As the age increases,the number of accidental injuries at home / residence increases gradually.There was statistically significant difference between different gender and age groups(P(27)0.05).5.The sequence of injury nature included fracture(68.11%),head trauma(15.64%),superficial injury(11.32%),joint or soft tissue injury(2.06%),organ injury(1.65%),multiple trauma(1.03%),poisoning(0.21%).The difference between genders was statistically significant(P(27)0.05).6.The sequence of injury sites was lower extremity(44.86%),trunk(20.16%),head(16.67%),upper limb(12.96%),other(4.73%)and multiple(0.62%).Lower limbs are the main injured parts of all ages.There were significant differences between different genders and age groups(P(27)0.05).7.Of all cases in the shortest duration of hospitalization was 0.5 days,the longest duration of hospitalization was 157 days,the median duration of hospitalization and the four percentile interval is 9 days;the lowest hospitalization expenses for 582.80 yuan,the highest 238345.60 yuan for hospitalization,the median hospitalization expenses and four percentile interval were 9002.15 yuan and 3525.28-25794.56 yuan.The patients with higher average daily hospitalization expenses were female,60~64 and 70~74 years old group and joint or soft tissue injury cases.8.Different causes of injury,injury and damage properties of parts in the length of hospital stay and hospital costs are different,the material cost in the total cost of hospitalization in the highest proportion,22 single factor analysis results indicate that the 60 to 64 year old age group,retirement,fracture,and lower limb surgery,two or more hospitals,hospital stay more than 9 days the total hospitalization expenses were higher in male than female high material cost;multi factor analysis showed that the fracture of head trauma and higher hospital costs,fracture surgery,high material cost,there are two hospitals and hospital stay more than 9 days of hospitalization cost and material cost were higher.Conclusion:1.Due to accidental injury in the elderly,more women than men,riding the electric car / bike and walking is a major cause of accidental injury,falls and traffic accidents were the main damage type,lower limbs and head is more and more serious injury site,fracture is the main form of damage.2.Different causes of injury,injury and damage properties of parts in the length of hospital stay and hospital costs are different,the material cost in the total cost of hospitalization in the highest proportion,22 single factor analysis results indicate that the 60 to 64 year old
