Objetive: To explore a novel cross-link configuration for combination with the posterior C 2 pedicle screw system in treatment of unstable Hangman's fracture.Method: Collected from February 2011 to April 2015 in the PLA 101 hospital underwent the operation treatment and the clinical data of 13 cases of unstable Hangman fracture with the self-made transverse neck and 2 pedicle screw system.To analyze the patients ' operation time,blood loss,follow up the X-ray film or CT to observe the fracture reduction and healing,the clinical effect standard of fracture treatment and the neck visual analogue pain score.Results: After the postoperative follow-up of 12-36 months,the average follow-up(22.3±8.5)Month,surgery time(66.5±14.1)min,intraoperative bleeding(77.3±14.7)ml.There were no important vascular and nerve injuries during operation,no cerebrospinal fluid leakage,hematoma formation and wound infection were observed.The fracture site was normal bone healing in the March after the operation,and the nail rod was in good condition and had no displacement and slippage.Preoperative neck Pain VAS Score(6.8±1.2),the last follow-up was(1.2±0.4),the difference was statistically significant(p<0.05);Evaluation of clinical efficacy in 13 cases of patients after the last follow-up: excellent 11 cases,good 2 cases,excellent rate of100%,average score(38.2±2.6)is excellent.Conclusion: The treatment of unstable hangman's fracture by homemade transversepedicle screws can increase the immediate stability of the fracture,simplify the operation,lower the incidence of complication and be a physiological reconstruction fixation method,and can get a satisfactory clinical effect.
