Research On The Commonweal Evaluation Of County-level Public Hospitals Based On The PATH Project

Background: In April 23,2015,the opinion about the State Council on the full implementation of the county-level public hospital comprehensive reform adheres to the basic orientation on public welfare of public hospitals.It has become an important goal of deepening the reform of public hospitals that the government try to achieve the public welfare of public hospitals under the new environment of medical reform.At present,our country is in a critical period of medical and health system reform.The government ensures the essence and developmental direction,adheres to public welfare as significant goal in the reform of public hospitals by means of financial investment,preferential tax policies and policy guidance.The purpose of this study is to build a performance evaluation index system based on PATH Project built by the previous research group,and to carry out the public welfare evaluation of county public hospitals in Fujian,Anhui and Qinghai provinces based on the "public welfare oriented" dimension.The study aims to evaluate the detailed changes and differences between original situation and reformed situation on public welfare degree.It also explores the main indicators of public welfare level,the influencing factors and fundamental causes of public welfare degree.The study is to provide feasible policy suggestions so as to further improve the public welfare of public hospitals at the county level.Methods:1.Using the method of literature review and expert consultation,and the specific method of the importance of public connotation,analysis of public hospital evaluation,on the basis of the design including "public welfare oriented" patient satisfaction survey,the county public hospital performance evaluation questionnaire and interview outline;2.Aata processing and analysis methods: Epi Data 6 to establish the database using all the questionnaire data entry,double double machine to ensure the data quality;quantitative data using SPSS 16 statistical software for descriptive analysis,comparative analysis of the present situation and trend of Fujian,Anhui and Qinghai Province,the county public hospital public welfare,study the main factors affecting the public welfare;the application and analysis of the index system by TOPSIS method,the public welfare of public hospitals to evaluate the degree of realization.Qualitative data were used to evaluate the public welfare status of public hospitals by classification and corresponding correlation analysis,so as to provide theoretical support for discussions and suggestions.Conclusion:(1)There is a positive correlation between the level of average cost and the degree of economic development(2)The compensation ratio of medical insurance inpatients showed an increasing trend in East and West of China(3)There are regional differences in the completion of government mandatory tasks(4)The use of basic drugs can promote the realization of the hospitals' commonweal(5)The level of public welfare is higher in Western China than in the eastern and central regions.Suggestions:(1)Control the growth of sub average costs,increase monitoring,and establish rewards and punishments(2)Promote the three process of maintaining the one,improve the fund's payment ratio and give full play to the leverage of medical insurance(3)Update the catalogue of basic drugs in time to ensure the sustainability of the use of basic drugs(4)Under the new medical reform environment,various forms ensure the public welfare of medical institutions...
