Objective:To compare the therapeutic effects of ultrasound-guided diaphragmatic nerve block(ultrasound guided phrenic block)on intractable hiccup(intractable hiccup,IH)by comparing with the blind-method of phrenic nerve blockade(phrenic blocks),It provides a theoretical reference for the treatment of IH by using ultrasound-guided diaphragm nerve block technique in clinic.Methods:According to the diagnostic criteria of 48 hours of hiccup,the 40 patients who were treated by pain department in the hospital of Yanbian University in October 2015 were randomly divided into a and B groups and 20 patients in each group.Patients in Group A were treated with 1%lidocaine 10ml ultrasound guided phrenic nerve block in patients with IH;B group using 1%lidocaine 10ml to treat IH.Two groups of patients treated 1 times a day,7 days for 1 courses,all treatment 1 courses.During the treatment of the two groups of patients with the same routine,all oral chlorpromazine tablets,1 times 1 tablets,the day(if necessary to increase the dosage).The criterion of curative effect is according to the standard of cure improvement of clinical disease diagnosis,The cure is that the hiccup symptom disappears completely,the improvement for the patient is reduced consciously,the number of hiccups is decreased,and the number of spontaneous hiccup is not decreased,and the symptom has no obvious improvement.The cure rate plus the improvement rate is always efficient.The efficiency and complications of the two groups in treatment of 1 courses(7 days),1 months and 3 months were compared.The measurement data are expressed by x±s,and the average number of the groups is compared with the card-square test.P<0.05 is statistically significant for the difference.Results:1.In group A,the success rate of nerve block was 97.9%,and in group B was 75%.The power of group A was higher than that of group B,but the difference was statistically significant(P<0.05).2.After one course of treatment,the effective rate in group A was 100%,and the effective rate in group B was 70%.One month after treatment,the effective rate in group A was 95%,and the effective rate in group B was 65%.The effective rate of group A was significantly higher than that of group B,and the difference was statistically significant(P<0.05).And after 1 months,the effective rate of patients in Group A was 65%in the 95%,b group,compared with Group A in the two groups,the group A was significantly higher than the 95%,B Group B,The difference was statistically significant(P<0.05).After 3 months of treatment,the effective rate of group A was 90%,the effective rate of patients in Group B was 60%,the Group A in the two groups was significantly higher than Group B,and the difference was statistically significant(P<0.05).3.There were no complications and drug allergy in Group A of patients during the treatment period,there were 3 cases of hematoma,nerve injury in 2 cases,no other complications were found.Conclusion:1.The success rate of ultrasound-guided radial nerve block was significantly higher than that of blind nerve block.2.Ultrasound-guided sacral nerve block treatment of IH was more effective than blind-sacral sacral nerve block after one course of treatment,one month,and three months later.3.Ultrasound-guided Phrenic nerve block is more safe for the treatment of IH than the blind method of phrenic nerve block,and the probability of complication is lower.
