Part?Fisher discrimination of MSCT features in GGN sample Lung adenocarcinomaObjective: to investigate the use of Fisher discriminant function model that is established on the basis of CT morphological featuresfor Lesion before infiltration and invasive lesions of the ground-glass nodules(ground-glass nodule,GGN)sample early lungadenocarcinoma.Methods: a retrospective study was conducted in 160 case of GGN sample early lung adenocarcinoma.These cases in Jingzhou central hospital and Dongfeng general hospital from September 2014 to February 2017 were confirmed by pathology.The CT morphological features were evaluated by chi-square test and Fisher discriminant was applied to parameter with significant difference,determine critical value Zc.Crosstabulation was used for estimating the rate of miscarriage justice.Results:160 patients were divided into two groups according to pathological results,the lesion before infiltration group(n=27),invasive lesions group(n=53),spicule signs,lobulation sign,nodules form,nodule interface,average diameter?8mm,pleural indentation sign,vascularconvergencesign and size of solid components were significant difference.The formula of fisher discriminant Z=2.046X1+2.006X2+2.015X3-1.473X4+0.189X5+0.023X6+1.813X7+1.957X8-1.687,determine the critical value Zc=0.According to the formula,there were 59 cases with Zi ? 0(preinvasive lesions)and 101 cases with Zi ? 0(invasive lesions),diagnosticsensitivity was 96.2%,specificitywas 92.5%,misdiagnosis rate was 7.5%,omission diagnostic rate was 3.8%,the total coincidence rate was 93.7%,Kappa coefficient was 0.863,Diagnostic level of consistency is "very good".Conclusion: Fisher discriminant mode for differential diagnosis GGN sample early lung adenocarcinoma before infiltration and invasive lesions with the clinical practical value.Part ?Correlation between MSCT features with EGFR mutations,Ki-67 expression in GGN sample Lung adenocarcinomaObjective:To discuss the correlation between MSCT features with EGFR mutation,Ki-67 expresstion in GGN sample lung adenocarcinoma.Methords: To detect the EGFR gene mutation and Ki-67 expression in tumor tissue of 160 cases GGN sample lung adenocarcinoma and 86 cases SPN lung adenocarcinoma.First,analyzed the differences of EGFR mutation,Ki-67 expresstion between GGN group SPN group.Divided the 160 GGN cases into several groupsaccording to its MSCT featuers within,without).Analyzed the differences of EGFR mutation and Ki-67 expression in the groups by using SPSS 19.Result: The expression of EGFR in the tumor tissue had statistical significance with pleural indentation,nodule type(P?0.05).It had no statistical significance with GGN average diameter ? or <8mm,the tumor-lung boundary was clear or blurred,and the burr sign,the lobulated,the vacuole sign within or without.And the high expression of Ki-67 had statistical significance with tumor size and nodule type(P ? 0.05),had no statistical significance withthe tumor-lung boundary were clear or fuzzy,and the burr sign,the lobulated,the bubble sign within or without.The Ki-67 high expression rate of EGFR mutation group was higher than that without EGFR mutation group,but the EGFR mutation was not correlated with Ki-67 high expression.Conclusion: Tumor size,pleural indentation and nodular type of lung adenocarcinoma had guiding significance in judging the EGFR mutation,Ki-67 expression.
