Objective:To observe the clinical efficacy,cardiac function and serum neuroendocrine factors(ALD,BNP)of continuous volume hemofiltration with different volume in the treatment of refractory heart failure,and explore effective treatment for refractory heart failure.Methods:We retrospectively analyzed 98 patients(55 males and 43 females)with refractory heart failure who were admitted to the department of intensive care in our hospital from June 2014 to June 2016.There was no statistical difference in gender and age between the two groups.Meaning.Ninety-eight patients were divided into two groups,A and B.Among them,46 patients with refractory heart failure who had a low volume filtration rate(rate 2 L/h)were included in group A,and 52 patients underwent high-capacity filtration rate(Refractory heart failure patients with a rate of 3/4L/h)were included in Group B.Both groups did not undergo any treatment for heart failure before admission.After admission,both groups received conventional drugs before hemofiltration treatment.Cardiac,diuretic,vasodilator,etc.were used to compare clinical efficacy,cardiac function,and neuroendocrine factors(ALD,BNP)after treatment in the two groups.The T test was used to calculate the difference between two independent samples.Differences between the count data were performed in a chi-square test,and P<0.05 was considered statistically significant.Results:After statistical analysis,two groups of patients before treatment and monitoring indicators such as LVEF,MAP,24h,respiratory rate,urine volume,HR,CI,Pa02/Fi02,ALD,BNP,neuroendocrine factors,and patients with APACHE ? score of the observation indicators were no significant difference,P values were greater than 0.05;two groups of patients after treatment the observed indexes before treatment were improved,P values were less than 0.05,and the improvement of indexes in group B was more significantly,the difference was statistically significant,P<0.05;B group of patients with total efficiency was significantly higher than that of A group(84.8%VS94.2%),the difference has statistical significance(P<0.05).Conclusion:Continuous veno venous hemofiltration(CVVH)in the application in the treatment of patients with refractory heart failure,with the clinical curative effect caused by the different filtration rate is different,in general CVVH of patients with refractory heart failure is good,it has an important significance in the clinical course of treatment.Our study found that in the case of high volume filtration rate,the clinical efficacy,cardiac function and neuroendocrine factors(ALD,BNP)of patients were better than those of low volume rate group,providing a basis for clinical treatment of refractory heart failure.
