Research Objective:To investigate the effect of regular prenatal care on the pregnancy outcome of severe preeclampsia.To explore differences in expectant management time and the best timing and manner of termination of pregnancy in early onset severe preeclampsia.To achieve a purpose of realizing the importance of regular prenatal care and improving pregnancy outcome in Severe Preeclampsia.Research Methods:A total of 320 patients with severe preeclampsia at Yangzhou Subei Hospital from April 2013 to May 2017 were retrospectively.1.320 patients were divided into study group(with regular prenatal care)and control group(with irregular prenatal care)according to the number of prenatal examinations,pregnancy characteristics,pregnancy outcomes,perinatal outcome were compared between two groups.2.159 patients with early onset severe preeclampsia were grouped according to the time of onset;expectant management time and the timing of termination of pregnancy were compared among different groups.3.159 patients with early onset severe preeclampsia were grouped according to the time of pregnancy termination,pregnancy outcomes and perinatal outcome were compared among different groups.4.20 patients who delivered before 32 weeks were divided into group A(with cesarean section)and group B(with spontaneous vaginal delivery)according to the manner of termination of pregnancy,pregnancy outcomes and perinatal outcome were compared between two groups.Research Results:1?Among the 320 cases,193 cases were regularly scheduled(study group),and other 127 cases were not regularly scheduled(control group).In study group,the MaxSBP and MaxDBP were lower than the ones in control group,the average age and the time of pregnancy termination were larger than the ones in control group,the Parturient ratio was higher than the ones in control group.Differences were statistically significant(P<0.05).The incidence of placental abruption,postpartum hemorrhage,damage of liver and kidney,heart failure and eclampsia in study group were lower than that in control group.Differences were statistically significant(P<0.05).The incidence of FGR,abnormal vasa umbilical,mild asphyxia,severe asphyxia in study group was lower than that in control group.Differences were statistically significant(P<0.05).2?159 patients with early onset severe preeclampsia were divided into 3 groups according to the time of onset.Group A(gestation week less than 30 weeks)49 cases,group B(gestation weeks between 30 and 31+6 weeks)51 cases,group C(gestation weeks between 32 and 33+6 weeks)59 cases.Compared the average of delivery pregnancy week among three groups,groupC>groupB>groupA.Compared management time among three groups,groupB>groupA>groupC.Differences were statistically significant(P<0.05).3?159 patients with early onset severe preeclampsia were divided into 3 groups according to the time of pregnancy termination.Group A(pregnancy termination weeks less than 32 weeks)20 cases,group B(pregnancy termination weeks between 32 and 33+6 weeks)56cases,group C(pregnancy termination weeks more than 34weeks)83cases.Analyze the incidence of serious pregnancy complications,including placental abruption,postpartum hemorrhage,damage of liver and kidney,HELLP syndrome,heart failure and eclampsia,groupA>groupB>groupC.Differences were statistically significant(P<0.05).The perinatal mortality among the four groups was almost the same(P>0.05).Analyze the incidence of neonatal asphyxia,groupA>groupB>groupC.Differences were statistically significant(P<0.05).4?20 patients who delivered before 32 weeks were divided into group A(with cesarean section)and group B(with spontaneous vaginal delivery)according to the manner of termination of pregnancy.The,incidence of serious pregnancy complications,including placental abruption,postpartum hemorrhage,damage of liver and kidney,HELLP syndrome,heart failure and eclampsia between the two groups was almost the same(P>0.05);the incidence of neonatal asphyxia between the two groups was almost the same(P>0.05).Perinatal mortality in group B(with spontaneous vaginal delivery)was higher than the ones in group A(with cesarean section).Differences were statistically significant(P<0.05).Research Conclusion:1?Through regular prenatal care,the symptoms of severe preeclampsia can relieve,the termination of pregnancy can delay,serious pregnancy complications and adverse outcomes of perinatal infants can reduce.So regular prenatal care can improve pregnancy outcome of severe preeclampsia.2?Patients whose time of onset between 30 and 31+6 weeks,their management time is the longest.3?When the gestational week reaches 34 weeks and more,the incidence of serious pregnancy complications and neonatal asphyxia dramatic decline,perinatal mortality also decline.4?Delivered before 32 weeks,the incidence of serious pregnancy complications and neonatal asphyxia has no obvious difference between with cesarean section and with spontaneous vaginal delivery.
