Objective To analyze the risk factors of cerebral microbleeds(CMBs)among middle-aged and old people without cerebral infarction or cerebral hemorrhage and compare the difference of risk factors between mild,moderate and severe grade.To explore the distribution of lesions in the CMBs group.Method The inpatients admission to department of neurology in our hospital during January 1st 2015 to December 31 st 2017 were recruited to our study and divided into CMBs and Non-CMBs group by susceptibility weighted imaging(SWI).CMBs group were categorized into three subgroups including mild,moderate and severe grade by the quantity of lesions.We recorded the case with or without lacunar infarction(LI)and leukoaraiosis(LA)according to the magnetic resonance examination results.The general condition,case history,lipids,renal function,and glycosylated hemoglobin were collected.Independent sample t-test,c2 test,logistic regression analysis were used to identify the possible risk factors including age,gender,smoking,drinking,hypertension,LI,LA,lipids,plasma homocysteine,renal function,glycosylated hemoglobin.Lipids,plasma homocysteine,renal function,and hemoglobin A1c of different subgroups of CMBs group were analyzed by one-way ANOVA.The distribution of lesions in CMBs with different grades was analyzed by summation and frequency.Result In the 115 included cases,about 41 cases are CMBs,and 74 cases are Non-CMBs,and the morbidity of CMBs is 35.7%.The age distribution of included cases is 40 to 86,the average age of CMBs group is 64.17±10.205,and the average age of Non-CMBs group is 57.88±9.334.The hypertention cases of two groups respectively are 30 and 20.Between the two groups,age,hypertention,Ccr,Cys C and LI all show the significant variations(p<0.05).We use Binary classification to analyze age(OR =0.916)and hypertention(OR=0.086),and found that this the two factors are the independent risk factors of CMBs occurrence.In the CMBs group,there are 16 mild cases,10 moderate cases,and 15 severe cases.In the variates of this study,hypertention have significant difference(P<0.05),blood lipid,renal functionand Hb AIc not show remarkable discrepancy.Predilection site of focus are lobe,deep white matter of brain,brainstem,thalamus,and cerebellum in turn.Conclusion Age and hypertension are independent risk factors for the onset of CMBs when cerebral infarction and cerebral hemorrhage are not combined in middle-aged and elderly people over 40 years old.Hypertension has an impact on the severity of CMBs.
