Objectives Investigate the feasibility and correlation of 'H magnetic resonance spectroscopy(1H-MRS)on the quantification of hepatic and combined with clinical and blood biochemical indexes,In order to find another better evaluation method.To evaluate the clinical application value of 1H-MRS in quantitative detection of lipid content in non-alcoholic fatty liver.Materials and Methods By B-ultrasonic,the patients enrolled who meet the requirements were divided into two groups,one group with 29 normal subjects without fatty liver as control group,the other is the NAFLD group with 42 non-alcoholic fatty liver patients,then further divided into three groups:mild(22 case),moderate(8 case),severity(12case)based on intrahepatocellular lipid(IHCL),due to the small number of moderate group,finally moderate and severe group was combined into one group to meet statistical requirements.Before being scanned,physical parameters of BMI and waist circumference and blood biochemical laboratorial examinations were undergone in all of the 71 case,and went through liver magnetic resonance imaging(MRI),in-phase/opposed-phase(IN/OPP)and 1H-MRS for hepatic using a PHILIPS 3.0T magnetic resonance scanner,at the same time IHCL values determined by each technique was get.The values of water peak and lipid peak and areas under the water peak and lipid peak were measured on 'H-MRS.Ultimately,the physical parameters of BMI and waist circumference,biochemical results,IHCL was compared in our study.Statistic method:SPSS 16.0 was used to analyze the parameters.Results 1.Magnetic resonance spectroscopy curve for hepatic consistent with B-ultrasonic diagnosis.In the normal group there are 24 people only see towering water peak near the 4.7 ppm,and did not see lipid peak near the 1.3 ppm.Low fat peak can be seen in another 5 person near the 1.3 ppm.In mild and moderate severe group not only see water peak,but also see different levels of lipid peak near the 1.3 ppm.2.There was no significant difference in PW between the three groups(P>0.05).The three group of BMI,waist circumference,TG and TC was different,moderate severe group>mild group>normal group.There was significant difference in HLD between the three groups(P<0.05),moderate severe group<mild group<normal group.3.PL in moderate severe group is obviously higher in the mild group,so is AL and IHCL.There are significant difference between the two groups(P<0.05).4.In moderate severe group,TG and IHCL was well correlated with a spearman correlation coefficient of r,=0.677(P<0.05),so is TC.In moderate severe group,there was negative correlation between HDL and IHCL of rs=-0.661(P<0.05).5.IHCL and fatty liver grade was well correlated with a spearman correlation coefficient of rs =0.909(P<0.05),so is BMI,Waist circumference,TG and TC.There was negative linear correlation between HDL and fatty liver grade.Conclusions 1.1H-MRS can quantificat of hepatic steatosis,and has higher accuracy could be used as an important method for the quantitative assessment of fatty liver.It also can grade the severity of fatty liver.2.Combination of IN/OP and 1H-MRS improved the accuracy of the diagnosis of fatty liver.However,'H-MRS showed significant advantages in distinguishing normal and mild fatty liver.3.Blood biochemical indicators couldn't diagnose fatty liver.whereas,waist circumference,BMI and TG can predict the severity of fatty liver in some degree.
