Value Of MRCP,Common Bile Duct Diameter And Liver Function Test In Judging Preoperative Lc Common Bile Duct Stones And Preventing Bile Duct Injury

OBJECTIVE: To retrospectively analyze the diagnostic value of MRCP,common bile duct diameter and liver function test before laparoscopic cholecystectomy in the diagnosis of choledocholithiasis and bile duct anatomical variation,and to explore the significance of magnetic resonance cholangiopancreatography Cholangiography(MRCP),common bile duct diameter,liver function tests for laparoscopic cholecystectomy surgical options,to help guide the operation,reduce the risk of bile duct injury and residual stones in the common bile duct.Methods: A retrospective analysis of 103 patients with laparoscopic cholecystectomy undergoing ultrasound-guided cholecystolithiasis and cholecystitis in the Hepatobiliary Surgery at Dalian City Center Hospital from January 2016 to January2017 was performed retrospectively.Liver function tests,ultrasound and MRCP examination.First of all,MRCP,ultrasound examination of bile duct stones in patients with intraoperative confirmation of the situation is divided into two groups,compared MRCP,ultrasound diagnosis of common bile duct stones,?2test between groups compared MRCP,ultrasound examination of common bile duct stones Rate of difference is statistically significant.Second,based on whether abnormal liver function and whether the common bile duct diameter is divided into two groups,respectively,?2test to compare the abnormal liver function and common bile duct diameter and common bile duct stones in the diagnosis of whether there was statistical significance.According to MRCP examination,34 patients with choledocholithiasis were grouped according to the size of gallstones?0.5cm and> 0.5cm.Fisher test was used to compare the MRCP between the groups.The choledocholithiasis test Whether the rate is statistically significant,and analysis of MRCP detected bile duct mutation cases,for guiding the operation,reduce the clinical value of bile duct injury.Results;?2test between MRCP and ultrasound in the detection of common bile duct stones and the results of surgery,P <0.05 was statistically significant.Liver function abnormalities and common bile duct diameter cases and diagnosis of common bile duct stones between the ?2test,with statistical significance.For MRCP detected 34 cases of choledocholithiasis patients in accordance with the diameter of common bile duct stones to 0.5cm as the limit,the line between the ?2test,P <0.05,statistically significant,MRCP group found in 3 cases of hepatic duct,Mirizzi Syndrome in 1 case,confirmed by surgery,did not appear serious complications of bile duct injury.Conclusion: For patients with cholecystitis and cholecystolithiasis detected by ultrasound,if abnormal liver function is detected and the common bile duct diameter is widened,MRCP examination should be considered in combination with suspected choledocholithiasis,which not only can avoid the missed diagnosis rate of residual common bile duct stones,And can understand and find possible biliary systemvariations and types,which can be used as a preoperative judge of biliary system length,walking,distribution and import parts,guide the operation,reduce the damage of the bile duct has important clinical value.
