| Objective: ovarian malignant tumor(ovarian cancer)is currently the highest fatality rate in gynecologic malignant tumor malignant tumor,according to statistics,the United States in 2016 new cases a total of 22280 cases,14240 cases of death [1],China’s 2011 new cases of 83851 cases of [2].Due to the insidious onset of ovarian cancer and the lack of effective early diagnostic methods,about62 percent of patients are in the advanced stage of treatment[1].Standard treatment for patients with advanced ovarian cancer is surgery and systemic chemotherapy for paclitaxel combined with platinum.However,relapse after first-line treatment is common.Chemotherapy resistance is the main cause of recurrence and treatment failure of ovarian cancer.Despite the new drug resistance in chemotherapy,targeted therapy,and other treatment methods are studied,but in clinical outcome or the improvement of cure rate was not significant,the 5-year overall survival rate is only 45%.So the ovarian cancer in ovarian cancer treatment of postoperative chemotherapy drug resistance is one of the most serious problems facing the current medical level of ovarian cancer drug resistance is unpredictable,as the treatment of ovarian cancer to individualized treatment,looking for suitable predictor of the chemotherapy drug resistance and prognosis of ovarian cancer,will help to individualized treatment plan.At present,in the study of ovarian cancer,it is found that in 96% of serous ovarian tumors,a gene TP53 mutation is identified.TP53 encoding p53,a tumor suppressor,serves as the main control center for cell response to various pressures,including DNA chemotherapy.Once activated,p53 can be used as a sequence specific transcription factor or protein:protein interactions,initiating cell cycle arrest,cell apoptosis,and DNA damage repair to fight cancer.This experiment by immunohistochemical technique,analysis from the level of protein expression of the gene in human ovarian cancer tissue and the relationship between the ovarian cancer chemotherapy drug resistance,and to analyze the prognosis of ovarian cancer related factors,explore p53 protein in ovarian tumor and prognosis assessment.Methods:1 this study selected from January 2012 to December 2012,between the line in the center of dalian in dalian medical university affiliated hospital of department of gynaecology early ovarian cancer staging surgery or comprehensive satisfaction of tumor cells to destroy the loss,postoperative pathological specimens confirmed by pathology,the first hospital of ovarian epithelial carcinoma,all cases handled all the line in our platinum chemotherapy based on the specification,the screening of a total of 62 cases.2 will be more than 62 cases of follow-up,according to the related factors may affect the tumor recurrence and prognosis grouping,the correlation between the comparison groups,the understanding of ovarian cancer recurrence time,and to understand the clinical prognosis.3.Immunohistochemical techniques were used to detect the expression of P53 in epithelial ovarian cancer.4.SPSS 22 is adopted.0 statistical software,the chi-square test method was used to compare the expression of P53 protein in various related factors.Using COX regression to the data of patients with survival analysis,analysis of the age,clinical pathologic stage,numerical,lymph node metastasis,clinical stage,preoperative CAl25 p53 protein expression effect on the overall survival.The result of each group is a=0.As the test level,P<0.The differences were statistically significant.Results: p53 protein expression was found in 67.7% of patients.Chi square test indicated that the overexpression of p53 protein was positively correlated with the increase of serum CAl25 and FIGO staging(p<0).05);Single factor analysis indicated that the prognosis of patients was correlated with pathological grading,lymph node metastasis,clinical pathological stage,preoperative CAl25 level and p53 high expression(P=0.027,0.002,0.Multivariate analysis suggested that lymph node metastasis and p53 high expression were independent prognostic factors(P=0.042,0.022).Conclusions;1.P53 high expression is related to the chemotherapy resistance of epithelial ovarian cancer,which can be used to predict the efficacy of chemotherapy in patients.For patients with high drug resistance gene expression,individualized treatment can be given to avoid chemotherapy injury.2.The expression of p53 protein is closely related to the prognosis of advanced ovarian cancer,and can be used as an important prognostic indicator for ovarian cancer. |
