Application Of Hypotensive Resuscitation To The Treatment Of Hemorrhagic Traumatic Shock

Objective: To investigate the hypotensive resuscitation for the treatment of hemorrhagic traumatic shock,which can provide a strong basis for the decision of clinical treatment.Methods: Clinical datas of 98 patients with traumatic hemorrhagic shock were retrospectively analyzed whose treated in our emergency trauma center.According to different resuscitation modes,patients were divided into conventional positive resuscitation group(conventional group,fifty-six cases)and hypotensive resuscitation group(restricted group,forty-two cases).Comparison of different resuscitation group between preoperative infusion volume,blood coagulation function(PT and APTT),platelet,hemoglobin,base excess(BE),plasma lactic acid content,acute respiratory distress syndrome(ARDS),multiple organ dysfunction syndrome(MODS)and mortality incidence rate.Results:Compared with hypotensive resuscitation group,the prothrombin time(PT),activated partial thromboplastin time(APTT),and the lactic acid content of conventional positive resuscitation group increased obviously,and the platelet,hemoglobin and BE of conventional positive resuscitation group were markedly reduced.The mortality and the incidence of ARDS and MODS of hypotensive resuscitation group were declined obviously than conventional positive resuscitation group,the difference between the groups was statistically significant(P<0.05).Conclusion: Early hypotensive resuscitation can improve the coagulation function of patients,alleviate acidosis,and reduce the mortality and the incidence of ARDS and MODS,which can improve the prognosis for traumatic hemorrhagic shock patients.
