| Objective: To evaluate the value of ultrasound elastography and contrast-enhanced ultrasonography in the diagnosis of thyroid nodules by summarizing the related literatures of ultrasonic elastography and contrast-enhanced ultrasound in the diagnosis of thyroid nodules,and by meta analysis.Method: English set the key words of "thyroid nodule","ultrasonoelastograph","contrast.enhanced ultrasound" Pubmed,MEDLINE to search " thyroid nodules,contrast-enhanced ultrasound,ultrasound elastography" for the retrieval words China CNKI,Wanfang database,weipu database,using the fuzzy retrieval before covering published on July 2017 by ultrasound angiography and ultrasound elasticity technique in the diagnosis of thyroid nodules in literature.In the literature there must be a biopsy as the gold standard,the article can obtain respectively two examination for thyroid nodule diagnosis results of true positive,false positive(TP)(FP)(TN),true negative,false negative(FN)data of four data table,into the heterogeneity test documents with diagnostic ratio than,get the combined sensitivity and diagnosis of two kinds of examination of the combined effect of the amount of the merger with specificity,positive likelihood ratio,and negative likelihood ratio,combined with meta analysis,diagnostic odds ratio and 95% confidence interval I;summary receiver operating characteristic(summary receiver operating characteristic curve curve,SROC),the calculation.The area under the curve(AUC),the ultrasound elasticity imaging and contrast-enhanced ultrasound in diagnosis of thyroid nodule diagnosis effectiveness evaluation.Result: For the first time,87 Chinese literatures and 6 English literatures were collected.8 abstracts and 1 Chinese literatures were collected by reading abstracts and contents,and 914 English nodules were included.Heterogeneity test: sensitivity calculation and logarithm of(1-specificity)Spearman logarithmic correlation coefficient P,P value of ultrasound elastography and contrast enhanced ultrasound in 0.67 P 0.22: DOR effect,ultrasonic elastography and contrast enhanced ultrasound with Cochran-Q value of 30.91 and 21.42,the P value is 0.0001 and 0.006,the I value is 61.4 and 42.3;using a random effects model with diagnostic effect: ultrasound elastography combined with sensitivity and 95%CI is 0.87(95%CI0.83-0.90),with specificity and 95% CI is 0.88(95%CI0.85-0.91)and positive likelihood ratio and 95%CI is 6.30(95% CI3.75-10.59),and the negative likelihood ratio is 0.16 and 95%CI(95%CI0.09-0.26),and the pooled odds ratio 95%CI is 44.30(95%CI18.99-103.33);combined with ultrasound contrast sensitivity and 95%CI is 0.86(95%CI0.82-0.89),with specificity and 95%CI is 0.88(95%CI0.84-0.90)with The positive likelihood ratio is 5.91 and 95%CI(95%CI3.88-9.00),with a negative likelihood ratio and 95%CI is 0.18(95%CI0.13-0.25),the pooled odds ratio and 95%CI is 36.30(95%CI18.53-71.13);SROC curve showed that the ultrasonic elastography and contrast enhanced ultrasound Q statistic is 0.8721 and 0.8583,0.9357 and 0.9243 of the area under SROC curve.Sensitivity analysis selected two articles,subgroup analysis.Conclusion: In the diagnosis of thyroid nodules,the diagnostic efficacy of ultrasonic elastography in nodular>1cm is lower than that of contrast-enhanced ultrasound,and<1cm is superior to contrast-enhanced ultrasound in the diagnosis of thyroid nodules. |
