Objective:To compare the difference between Spleen-preserving distal pancreatectomy with splenic vessels preservation and distal pancreatectomy with splenectomy,evaluated the necessity and reliability of spleen-preserving in distal pancreatectomy.Methods:A retrospective analysis of patient case,the Second Hospital of Jilin University Hospital in January 2013 to January 2018 during the treatment in 33 cases of distal pancreatectomy.To compare the difference between 16 cases with spleen-preserving(group A)and 17cases without spleen-preserving(group B)by analysising the operation time,Intraoperative bleeding volume,time of hospitalization,major postoperative complications and changes of blood platelet count before and after operation.Results:The average operation time of Group A was 255.7±20.8min,and Group B was 172.6±15.1min,Group A taked a longer time(P=0.002).The average intraoperative bleeding volume of Group A was 143.6±32.4ml,and Group B was 207.3±41.3ml,Group A had less volume(P=0.006).The average time of hospitalization in group A was 13.0±1.9d,and group B was 19.9±2.3d,the hoshospital stay of Group A was shortened(P=0.001).The total incidence of major postoperative complications in Group A was12.5%,and Group B was 52.9%,Group A was significantly lower(P=0.01).The average blood platelet count in 1,2,4 weeks after surgery of Group A was 194.9±18.8×10~9/L,173.7±25.1×10~9/L,181.1±22.1×10~9/L,and Group B was 312.4±76.3×10~9/L?243.8±31.7×10~9/L?187.7±43.2×10~9/L,Group B was significantly higher in first and second week(P<0.01),but there was no significant difference in 4th week(P=0.06).Conclusions:Compared with distal pancreatectomy without Spleen-preserving,it is time consuming that spleen-preserving distal pancreatectomy with splenic vessels preservation for benign pancreatic lesions can reduce postoperative complications and shorten postoperative recovery time.
