| Objective:In order to evaluate the safety and effectiveness of low-profile guiding growth system.Through the spine animal model with young pig and combined with the imaging study:the distance of the proximal distal titanium rods with the spine growth sliding distance,the length of the T5-L3 spine,the posterior convex angle of the spine,the transverse longitudinal meridian of the thoracic spine,and the volume of the lung.It can induce the growth of spine,thorax and lung,and provide a theoretical basis for in-depth study of later scoliosis animal models and clinical application and promotion.Methods:1.To summarize the difficulties and challenges facing the treatment of early onset scoliosis,the technical characteristics and defects of the non fusion apparatus,a new low notch orthopedic system based on growth induction is designed and designed.The system consists of a low notch screw connected to a 5.5mm titanium rod,including a universal locking screw and a universal locking screw.Screw fixation and correction of spinal deformities;fish eye sliding screws with free rotation of the head joint,5.5 diameter titanium rods,titanium rods in the fish eye gliding head joint of the fish eye sliding screw and the free swaying of the sagittal plane and the free swing of the coronal surface in a certain range.Long.2.Animal experimental study:the total spinal CT scan was performed on 8 weeks old southern Yunnan small ear pigs before operation.The morphological image data of the spine of the young pigs were obtained,and the pedicle screw placement of the young pigs was guided.8 weeks old female healthy Southern Yunnan small ear pigs were randomly divided into three groups.Group A was a blank control group:T5-T6,T11-T12 and L2-L3 simple tissue dissection exposed joint process and no pedicle screw was implanted.Group B was a locking fixation group:bilateral T5-T6,T11-T12,and L2-L3 were implanted w:ith long tail pedicle screws,and a titanium rod was prebent and a screw was locked with a screw cap according to the sagittal plane of the spine before the operation.Group C growth induction group:new low notch growth induction system implantation,T5-T6 and L2-L3 bilateral implantation of fish eye nail,T11-T12 implantation of universal compression double ear nailing,and titanium rods preflexed by the sagittal plane of the spine,intermuscular wear,titanium rods fixed with double ear nail,and titanium rods free sliding in the fish eye nail and inducing the growth of the spine.After operation and March,the X-ray and CT examination of the spinal column of A,B and C were performed to measure the length of T5-L3 segment of the segment of the segment of the segment of the segment,the distance of the titanium rods at both ends of the head and the tail,the transverse diameter of the thoracic cavity,the longitudinal diameter and the volume of the lung.The changes of the coronary and sagittal Cobb angles of the experimental animals were observed,and the internal fixation of the spinal column was observed.Loss,there are no nails,broken nails,broken rods.Results:1.There was no significant difference in the length of T5-L3 spine,transverse diameter,longitudinal diameter and lung volume between the three groups of A,B and C animals(P>0.05).2.In group C in March,the average slip length of the titanium rod at the head end was 1.93 + 0.37cm,the average slip length of the tail end titanium rod was 2.4 ﹢0.36cm,and the average slip length of the titanium rod was 4.33 + 0.68cm.3.The length of T5-L3 segment of A group in group A was 24.74 + 0.58cm after operation,and the average increase was 4.87 + 0.74cm after operation.The length of T5-L3 segment in group B in March after operation was 19.99 + 0.55cm,and the length of T5-L3 segmental spine in group C was 24.07 + 0.44cm after operation,and the average increase was 4.21 + 0.59cm after operation.There was no significant difference in the length of spinal column in group T5-L3 of C group in group A(P>0.05)in March after operation(P>0.05),and the length of T5-L3 segment in group A and C group increased significantly in group A after operation,and the difference was statistically significant(P<0.05).4.In group A after operation,the Cobb angle of T7-T10 kyphosis was 15.5 + 0.56 degrees in March,and increased by 1 + 0.05 degrees after the operation.The angle of T13-L1 kyphosis Cobb angle was 16.5 + 0.74 degrees,which was 1.6 + 0.03 degrees higher than that after the operation.The Cobb angle of T7-T10 kyphosis in group B was 22.5 + 1.35 degrees in March after operation,which was 9.5 + 0.89 degrees higher than that after operation,and the Cobb angle of T13-L1 kyphosis was 29.8 +1.49 degrees,which was 16.3 + 0.77 degrees after the operation.The Cobb angle of T7-T10 kyphosis in group C was 14.3 + 0.37 degrees in March after operation,which was 0.8 + 0.02 degrees higher than that after operation,and the Cobb angle of T13-L1 kyphosis was 15.8 + 0.25 degrees,which was 1.5 + 0.04 degrees after the operation.In group B,T7-T10 and T13-L1 kyphotral Cobb were significantly increased in group A and C in group A and C(P<0.05),and there was no significant difference between group A and C(P>0.05)in group A and C.There was no significant coronal angulation in group A,group B and group C in March.5.In March,the thoracic transverse diameter of group A was 21.10 + 0.05cm,which was 9.35 + 1.89cm higher than that after operation,and the longitudinal diameter of thorax was 28.09 + 0.26cm,which was 4.48 + 1.59cm higher than that after operation.In March,the thoracic transverse diameter of group B was 17.88 +0.12cm,which was 6.15 + 1.45cm higher than that after operation,and the longitudinal diameter of the thoracic spine was 23.94 + 0.08cm,which was 0.31 +0.42cm higher than that after operation.In March,the thoracic transverse diameter of group C was 20.65 + 0.33cm,which was 8.89 + 2.15cm higher than that after operation,and the longitudinal diameter of the thoracic spine was 27.70 + 0.58cm,which was 4.11 + 1.98cm higher than that after operation.The thoracic transverse diameter and longitudinal diameter of group A and C in group A and group C were significantly increased in March after operation,and the difference was statistically significant(P<0.05),and there was no significant difference between the thoracic transverse diameter and longitudinal diameter of A and C groups(P>0.05).6.The lung volume of group A in group A was 757.4 + 10.7ml after three months,and increased by 342.2 + 15.4ml than that in post operation.The lung volume of group B was 485.3 + 14.3ml,70.1 + 10.7ml and 755.3 + 12.6ml in C group,and 340.5+ 13.5m after operation.In March,the lung volume of group A and group C increased significantly compared with group B,the difference was statistically significant(P<0.05);there was no significant difference between A group and C group(P>0.05).7.In March B and C two groups of experimental animals had no rupture,loosening and nail removal.Conclusion(s):1.A new low-profile guiding growth system can safely and effectively induce spinal growth,reduce the restriction of internal fixation to the growth of the spine,and avoid the need for repeated opening of the traditional growth rod technique.2.The new low notch growth induction system can effectively induce the development of the thorax and ensure the normal development of the lung. |
